Resumo
Introdução: A Assistência Farmacêutica (AF) é um componente estratégico para a integralidade do cuidado em saúde no Sistema Único de Saúde (SUS), a qual envolve um conjunto de ações voltadas à promoção, proteção e recuperação da saúde, com foco no acesso e uso racional de medicamentos. Entretanto, a gestão da AF ainda enfrenta múltiplos entraves que comprometem sua efetividade nos três níveis de atenção à saúde. Objetivo: Realizar uma revisão narrativa da literatura científica que descreve os principais desafios e oportunidades na gestão da Assistência Farmacêutica no contexto do SUS, com foco em planejamento, logística e avaliação de desempenho. Métodos: Tratase de uma revisão narrativa de literatura. Foram selecionados artigos publicados entre 2010 e 2024, nas bases SciELO, PubMed e LILACS, utilizando os termos “Assistência Farmacêutica”, “Gestão em Saúde” e “SUS”. Foram incluídas publicações em português e inglês que abordaram modelos de gestão da AF, avaliação de políticas públicas e experiências em diferentes esferas do SUS. Resultados: Os principais desafios identificados incluem falhas na seleção e programação de medicamentos, aquisição ineficiente, ausência de tecnologias da informação, e desarticulação entre esferas federativas. A rotatividade de profissionais e a carência de capacitação contínua também comprometem a sustentabilidade da AF. Por outro lado, experiências positivas incluem a implementação de sistemas informatizados (como Hórus), o uso de indicadores de desempenho e o fortalecimento das Comissões Intergestores Regionais (CIR). A integração da AF com a Atenção Básica, por meio de ações clínicas e educativas, tem sido uma tendência crescente e promissora. Conclusão: A gestão da AF no SUS é multifacetada e depende de articulação intersetorial, financiamento adequado e formação técnica especializada. A literatura aponta para a necessidade de maior planejamento ascendente, uso de dados para tomada de decisão e valorização da força de trabalho farmacêutica. A qualificação da gestão da AF é essencial para garantir o direito à saúde, reduzir desigualdades regionais e promover a efetividade do cuidado farmacêutico.

Abstract
Introduction: Pharmaceutical Care (PC) is a strategic component for comprehensive healthcare within the Brazilian Unified Health System (SUS), involving a set of actions aimed at health promotion, protection, and recovery, with a focus on access to and the rational use of medicines. However, the management of PC still faces multiple challenges that compromise its effectiveness across the three levels of healthcare. Objective: To conduct a narrative review of the scientific literature describing the main challenges and opportunities in the management of Pharmaceutical Care within the SUS, focusing on planning, logistics, and performance evaluation. Methods: This is a narrative literature review. Articles published between 2010 and 2024 were selected from the SciELO, PubMed, and LILACS databases, using the terms “Pharmaceutical Care,” “Health Management,” and “SUS.” Publications in Portuguese and English addressing PC management models, public policy evaluation, and experiences at different levels of the SUS were included. Results: The main challenges identified include failures in the selection and planning of medicines, inefficient procurement, lack of information technologies, and disarticulation among governmental levels. Staff turnover and the lack of continuing professional training also compromise the sustainability of PC. On the other hand, positive experiences include the implementation of computerized systems (such as Hórus), the use of performance indicators, and the strengthening of Regional Intermanagement Committees (CIR). The integration of PC with Primary Health Care through clinical and educational actions has been a growing and promising trend. Conclusion: The management of PC in the SUS is multifaceted and depends on intersectoral coordination, adequate funding, and specialized technical training. The literature points to the need for greater bottomup planning, the use of data for decisionmaking, and the recognition of the pharmaceutical workforce. Improving the management of PC is essential to guarantee the right to health, reduce regional inequalities, and promote effective pharmaceutical care.
