OBJETIVOS: O Brasil é um dos países mais desiguais do mundo, o que se reflete na distribuição da saúde. Portanto, é importante analisar como os benefícios de uma intervenção em saúde são distribuídos. Aplicamos a Análise de Custo-Efetividade Distributiva (ACED) para avaliar como a vacina materna contra o Vírus Sincicial Respiratório (VSR) afeta a distribuição dos benefícios e custos em saúde nas macrorregiões de saúde brasileiras. MÉTODOS: Uma ACED foi aplicada no nível das macrorregiões de saúde brasileiras para uma coorte de nascimentos durante o primeiro ano de vida, utilizando dados de 2023. As estratégias comparadas foram vacinação materna universal (VM) com RSVPreF versus nenhuma VM. Estimamos uma distribuição basal de expectativa de vida saudável (EVS) antes da implementação da VM e como ela mudou após a intervenção. Os custos de oportunidade foram estimados considerando os custos incrementais divididos pelo Limiar de Custo-Efetividade (LCE) para o Brasil (US$ 8.016). Os custos de oportunidade foram distribuídos considerando o tamanho da população em cada macrorregião de saúde. Os benefícios líquidos em saúde foram estimados como os DALYs evitados com a vacina contra o VSR, menos os custos de oportunidade da mesma. Também analisamos possíveis compensações entre custo-efetividade e equidade. Uma análise de sensibilidade foi realizada para explorar mudanças nos preços e na distribuição dos custos de oportunidade. RESULTADOS: A vacina materna contra o VSR é custo-efetiva em apenas quatro das 121 macrorregiões, o que significa que a intervenção resulta em um benefício negativo em saúde em 117 macrorregiões e na perda de 4.481 DALYs evitados. A perda líquida em saúde está bem distribuída ao longo das macrorregiões, e a intervenção melhora ligeiramente a distribuição da saúde. A melhoria na distribuição não é suficiente para compensar a perda no benefício líquido em saúde, o que significa que, independentemente da magnitude da aversão à desigualdade, a intervenção não seria escolhida. CONCLUSÕES: A intervenção apresenta um benefício negativo em saúde, e a distribuição dos benefícios em saúde apresenta uma ligeira melhora nas macrorregiões de saúde brasileiras.

Abstract
OBJECTIVES: Brazil is one of the most unequal countries in the world, which is reflected in the distribution of health. Therefore, is important to analyze how the benefits of a health intervention are distributed. We apply the Distributional Cost-Effectiveness Analysis (DCEA) to evaluate how the maternal Respiratory Syncytial Virus (RSV) vaccine affects the distribution of health benefits and costs through Brazilian health macro-regions.
METHODS: A DCEA was applied at the Brazilian health macro-region level for a birth cohort during the first year of life, using data from 2023. The strategies compared were universal maternal vaccination (MV) with RSVPreF vs. no MV. We estimated a baseline distribution of HALE before the MV was implemented and how it changed after the intervention. Opportunity costs were estimated considering the incremental costs divided by the Cost-Effectiveness Threshold (CET) for Brazil (US$ 8016). The opportunity costs were distributed considering the size of the population in each health macro-region. Net health benefits were estimated as DALYs averted with the MV, minus the opportunity costs of the MV. We also analyzed possible trade-offs between cost-effectiveness and equity. Sensitivity analysis was conducted to explore changes in prices and distribution of opportunity costs.
RESULTS: Maternal RSV vaccine is cost-effective only in four of the 121 macro-regions, which means that the intervention results in a negative health benefit in 117 macro-regions, and the loss of 4481 DALYs averted. The loss in net health is well distributed along the macro-regions, and the intervention slightly improves the distribution of health. The improvement in the distribution is not enough to compensate for loss in net health benefit, which means that no matter how big the aversion to inequality is, the intervention would not be chosen.
CONCLUSIONS: The intervention has a negative health benefit, and the distribution of health benefits has slight improvement in Brazilian health macro regions.
