Objetivo: A hepatite C é uma doença que afeta aproximadamente 180 milhões de pessoas em todo o mundo (OMS 2006) e é uma das principais causas de doença hepática crônica. A infecção pelo HCV progride para a forma crônica em 80% dos indivíduos infectados. Aproximadamente 20% evoluem para cirrose ao longo de 20 anos e, consequentemente, têm um alto risco de desenvolver carcinoma hepatocelular. Avaliamos a razão de custo-efetividade incremental (ICER) do peginterferon alfa-2a (40 KD) mais ribavirina (PEG+RBV) versus interferon alfa-2b mais ribavirina (IFN+RBV) no tratamento de pacientes com hepatite C crônica sob a perspectiva do pagador Brasileiro. Métodos: Um modelo de Markov foi desenvolvido para estimar o impacto clínico e econômico relacionado à incorporação do peginterferon alfa-2a (40 KD). Os estágios clínicos foram baseados na histologia hepática, formas de descompensação cirrótica, câncer de fígado e transplante hepático. Um painel Delphi foi realizado para avaliar os recursos médicos diretos relacionados a cada estágio clínico na hepatite C crônica, bem como os custos do tratamento com peginterferon alfa-2a (40 KD), interferon alfa-2b e ribavirina. A eficácia do tratamento com peginterferon alfa-2a (40 KD) foi obtida a partir de um ensaio clínico multicêntrico, controlado e randomizado envolvendo 1121 pacientes naïve com hepatite C crônica (Fried et al., 2002). O modelo compreende um horizonte de vida. Assumimos uma taxa de desconto de 3% para custos e resultados, de acordo com as recomendações internacionais (Gold et al., 1996). Uma análise de sensibilidade foi conduzida utilizando simulação de Monte Carlo de segunda ordem. Os parâmetros testados foram custos por estágio, custos de tratamento, taxa de desconto, taxa de resposta ao tratamento, taxa de inflação e distribuição inicial dos pacientes. Resultados: O ICER do PEG+RBV versus nenhum tratamento foi aproximadamente -R$62.521 por ano de vida ajustado pela qualidade (QALY) ganho. O ICER do PEG+RBV versus IFN+RBV foi aproximadamente -R$20.087 por QALY. Conclusão: O estudo sugere que o peginterferon alfa-2a (40 KD) e ribavirina são uma terapia dominante para o tratamento da hepatite C no sistema de saúde privado no Brasil.

Abstract
Objetive: Hepatitis C is a disease affecting approximately 180 million people worldwide (WHO 2006) and is one of the main causes of chronic hepatic disease. HCV infection progresses to chronic form in 80% of infected individuals. Approximately 20% progress to cirrhosis over 20 years and, consequently, a high risk of developing hepatocellular carcinoma population. We assessed the incremental cost-effectiveness ratio (ICER) of peginterferon alfa-2a (40 KD) plus ribavirin (PEG+RBV) versus interferon alfa-2b plus ribavirin (IFN+RBV) in the treatment of patients with chronic hepatitis C under the Brazilian payer perspective. Methods: A Markov model was built to estimate the clinical and economic impact related to the incorporation of peginterferon-alfa-2a (40 KD). Clinical stages were based on liver histology, forms of cirrhotic decompensation, liver cancer and liver transplantation. A Delphi panel was performed for evaluating the direct medical resources related to each clinical stage in chronic hepatitis C, as well as costs from treatment with peginterferon-alfa-2a (40 KD), interferon alfa-2b and ribavirin. Effectiveness of treatment with peginterferon-alfa-2a (40 KD) was obtained from a multicenter, controlled, randomized trial involving 1121 naive patients with chronic hepatitis C (Fried et al, 2002). The model comprises a life-time horizon. We have assumed a discount rate of 3% for both costs and outcomes according to international recommendations (Gold et al, 1996). A sensitivity analysis was conducted using second-order Monte Carlo simulation. Tested parameters were costs per stage, treatment costs, discount rate, response rate to treatment, inflation rate and early patient distribution. Results: The ICER of PEG+RBV versus no treatment was approximately -R$62,521 per quality-adjusted life year(QALY) gained. The ICER of PEG+RBV versus IFN+RBV was approximately -R$20,087 per QALY. Conclusion: The study suggests peginterferon alfa-2a (40 KD) and ribavirin to be a dominant therapy for treating hepatitis C in the private health care system in Brazil.
