Resumo
Objetivo: Atualmente, pacientes com leucemia mieloide crônica (LMC) que são resistentes ou intolerantes ao imatinibe têm terapias minimamente eficazes disponíveis. O dasatinibe se liga à proteína Bcr-Abl; ele se liga também às formas ativa e inativa da proteína, enquanto o imatinibe se liga apenas às formas inativas. Portanto, mutações que afetam a forma ativa podem levar à resistência ao imatinibe. Um modelo Markov foi construído para avaliar a relação custo-efetividade do dasatinibe no tratamento de pacientes adultos com LMC, após resistência ou intolerância ao imatinibe. Métodos: O modelo consiste em um período inicial de ensaio em que as melhores taxas de resposta observadas nos ensaios clínicos são utilizadas. Resposta foi definida como a melhor resposta de resposta hematológica completa (CHR), resposta citogenética menor (CyR), CyR mínima, CyR parcial e CyR completa. O modelo simula pacientes transitando entre estados de saúde usando probabilidades de progressão derivadas da literatura e ensaios clínicos da BMS. O horizonte de tempo foi a vida inteira dos pacientes na coorte, permitindo a avaliação da expectativa de vida e dos custos ao longo da vida. Custos e estimativas de recursos de saúde Brasileiros foram aplicados ao tratamento das diferentes fases da LMC. Resultados: Para pacientes com LMC em CP, o dasatinibe proporcionou 0,66 QALYs por paciente e o ICER foi de R$80.000 com uma expectativa de vida adicional de 0,98 anos. No caso de AP, o dasatinibe proporcionou uma expectativa de vida adicional de 3,48 anos com um ICER de R$91.000. E no caso de BP, o dasatinibe proporcionou uma expectativa de vida adicional de 1,91 anos com um ICER de R$123.000. Conclusão: A análise de custo-efetividade mostrou que o dasatinibe é mais custo-efetivo em pacientes resistentes ou intolerantes do que o imatinibe nas três fases da LMC, com aumento da expectativa de vida com qualidade. Embora haja um custo incremental associado ao tratamento com dasatinibe, o custo está relacionado à maior expectativa de vida e, portanto, ao gasto de mais recursos.

Abstract
Objetive: Currently imatinib resistant or intolerant CML patients have minimally effective therapies available. Dasatinib binds to the protein Bcr-Abl; it binds also to active and inactive forms of protein, while imatinib binds only to the inactive forms. Therefore, mutations that affect the active form can lead to resistance to imatinib. A Markov model was built to evaluate the long-term cost-effectiveness of dasatinib in the treatment of adult CML patients, after resistance or intolerance to imatinib. Methods: The model consists of an initial within-trial period in which best response rates observed from the clinical trials are used. Response was defined as best response of complete hematologic response (CHR), minor cytogenetic response (CyR), minimal CyR, partial CyR, and complete CyR. The model simulates patients moving between health states using progression probabilities derived from the literature and BMS clinical trials. The time horizon was the lifetime of patients in the cohort, allowing evaluation of life expectancy and lifetime costs. Brazilian costs and health resource estimates were applied to the treatment of the different phases of CML. Results: For CML patients in CP dasatinib provided 0.66 QALYs per patient and the ICER was R$80,000 with an additional life expectancy of 0.98 years. In the case of AP dasatinib provided an additional life expectancy of 3.48 years with a ICER of R$91,000. And in the BP dasatinib provided an additional life expectancy of 1.91 years with a ICER of R$123,000 Conclusion: The CE analysis showed that dasatinib is more cost-effective in the resistant or intolerant patients than imatinib in the three phases of CML with increased life expectancy with quality. Though there is an incremental cost associated to the treatment with dasatinib, the cost is related to longer life expectancy and therefore expenditure of more resources.
