Resumo
Objetivos: Comparar os custos e a eficácia de dabigatrano etexilato (DAB) versus varfarina (WAR) em pacientes com fibrilação atrial não valvular (FANV) sob a perspectiva dos sistemas de saúde privados e públicos no Brasil. Métodos: Foi construído um modelo de Markov para comparar DAB com WAR e derivar a razão de custo-efetividade incremental (ICER) de DAB, baseado na literatura internacional e em um painel Delphi modificado com especialistas Brasileiros (padrão de prática clínica local no gerenciamento de pacientes com FANV). O modelo estimou o número de acidentes vasculares cerebrais isquêmicos e hemorrágicos, embolias sistêmicas, hemorragias intracranianas, ataques isquêmicos transitórios, hemorragias extracranianas, sangramentos menores e infartos agudos do miocárdio associados aos respectivos tratamentos. Para cada evento clínico foram atribuídos custos, incapacidades e/ou redução na qualidade de vida, e risco de morte. Apenas custos médicos diretos foram considerados e uma taxa de desconto de 5% foi assumida, de acordo com as diretrizes Brasileiras de HTA. Uma análise de sensibilidade probabilística foi projetada para avaliar a incerteza. Resultados: Sob ambas as perspectivas, pública e privada, DAB foi associado a um ganho adicional de 0,30 anos de vida (LY), 0,35 QALYs e demonstrou uma menor incidência de eventos intracranianos em comparação com WAR, resultando em custos menores com eventos e acompanhamento. O ICER para DAB versus WAR foi de R$ 39.740/LY e R$ 34.867/QALY na perspectiva pública e de R$ 25.252,48/LY e R$ 22.160,20/QALY na perspectiva privada. Análises de sensibilidade confirmaram a custo-efetividade de DAB. Conclusões: Os resultados sugerem que DAB pode ser custo-efetivo para prevenção de AVC quando utilizado em vez de WAR em pacientes com FANV no Brasil, considerando que os ICERs estavam abaixo do limiar de outras tecnologias reembolsadas.

Abstract
Objetives: To compare costs and effectiveness of dabigatran etexilate (DAB) versus warfarin (WAR) in patients with Non-Valvular Atrial Fibrillation (NVAF) from a private and public health care system perspective in Brazil. Methods: A Markov model was built to compare DAB versus WAR to derive the incremental cost effectiveness ratio (ICER) of DAB, based on the international literature and a modified Delphi panel with Brazilian experts (local clinical practice pattern on the management of NVAF patients). The model estimated the number of ischaemic and haemorrhagic strokes, systemic embolisms, intracranial hemorrhages, transient ischaemic attacks, extracranial hemorrhages, minor bleeds and acute myocardial infarctions associated with the respective treatments. To each clinical event costs, disabilities and/or reduction in quality of life, and risk of death were assigned. Only direct medical costs were considered and a discount rate of 5% was assumed, according to Brazilian HTA guidelines. A probabilistic sensitivity analysis was designed to assess uncertainty. Results: Under both, the private and public perspective, DAB was associated with additional 0.30 life years gained (LY), additional 0.35 QALYs and demonstrated a lower incidence of intracranial events versus WAR, resulting in lower event costs and follow-up costs. The ICER for DAB versus WAR was R$ 39,740/LY and R$ 34,867/QALY from the public and R$ 25,252.48/LY and R$ 22,160.20/QALY from the private perspective. Sensitivity analyses confirmed the cost-effectiveness of DAB. Conclusions: Findings suggest that DAB can be cost-effective for stroke prevention when used instead of WAR in NVAF patients in Brazil, given that the ICERS were below the threshold of other technologies reimbursed.
