Resumo
Introdução (com o objetivo): Os medicamentos podem apresentar riscos e efeitos adversos que exigem acompanhamento rigoroso. Apesar dos benefícios que o medicamento pode trazer, é possível que apareçam alguns efeitos indesejáveis, como as reações adversas. Nesse cenário, a farmácia clínica atua como um pilar indispensável de cuidado, com foco na segurança, recuperação da saúde e bem-estar do paciente. A presença ativa do farmacêutico clínico no ambiente ambulatorial promove uma assistência colaborativa que reduz erros de medicação, custos e mortalidade. Contudo, o acompanhamento presencial nas consultas farmacêuticas configura um desafio. Fatores socioeconômicos e a sobrecarga do sistema de saúde, atuam como barreiras que comprometem não apenas a adesão ao tratamento, mas também o estabelecimento de um vínculo efetivo entre o profissional e o núcleo familiar. Objetivo: Avaliar o perfil e a frequência das teleorientações farmacêuticas, bem como sua contribuição para a adesão, segurança e efetividade do tratamento dos pacientes acompanhados. Descrição: Estudo retrospectivo, descritivo e quantitativo com dados de 01/2025 a 12/2025, obtidos de prontuários de pacientes acompanhados pela farmácia clínica ambulatorial do ICr-HC-FMUSP via teleorientação por celular institucional, com consentimento do responsável. As teleorientações foram realizadas através do celular corporativo, fornecido pela Instituição. O contato foi realizado através de aplicativo de mensagem com autorização prévia do responsável, com o intuito de acompanhar o tratamento farmacoterapêutico. Em 2025, foram contabilizadas 364 teleorientações. O acompanhamento de adesão predominou, com 207 (56,87%) registros voltados ao monitoramento do cumprimento do plano de tratamento estabelecido em consulta. As orientações farmacêuticas totalizaram 110 (30,22%) das teleorientações, sendo a maior parte motivada por dúvidas ou ajustes no modo de uso (30%), o que engloba instruções sobre posologia e administração. Monitoramento da frequência e absenteísmo, por falta de retirada de medicamentos na farmácia (20,91%) e as substituições de itens da prescrição (15,45%), decorrentes de necessidades clínicas ou logísticas, também apresentaram volume expressivo. Além disso, observaram-se inclusões de tratamentos (9,10%), manejo de reações adversas aos medicamentos (9,10%), fundamentais para a segurança do paciente. Foram registradas falhas de adesão (3,63%), agravos à saúde (3,63%) e erros de prescrição (3,63%), prevenindo potenciais eventos adversos, demandas administrativas paralelas que impactam o tratamento do paciente (4,55%). Complementarmente, realizaram-se 42 (11,54%) doses assistidas, com supervisão direta da administração do fármaco e 5 (1,37%) ações de educação farmacêutica, visando aumentar a autonomia do paciente. Conclusão: O estudo destaca que a teleorientação não substitui o atendimento presencial, mas atua como complemento estratégico, especialmente no seguimento de pacientes crônicos e na resolução de demandas pontuais. Os resultados apresentados reforçam que essa modalidade contribui para a melhoria da qualidade do cuidado, otimização de recursos e fortalecimento da assistência farmacêutica clínica. Predominaram ações de adesão (56,87%), evidenciando o papel do farmacêutico na efetividade e prevenção de PRM. A teleorientação permitiu monitoramento, identificação de falhas, eventos adversos e erros. Demandas sobre uso e absenteísmo indicam fragilidades mitigáveis, reforçando educação, vínculo e segurança do paciente.

Abstract
Introduction (with the objective): Medications can present risks and adverse effects that require rigorous monitoring. Despite the benefits that medication can bring, some undesirable effects, such as adverse reactions, may appear. In this scenario, clinical pharmacy acts as an indispensable pillar of care, focusing on the safety, recovery of health, and well-being of the patient. The active presence of the clinical pharmacist in the outpatient setting promotes collaborative care that reduces medication errors, costs, and mortality. However, in-person follow-up in pharmaceutical consultations presents a challenge. Socioeconomic factors and the overload of the health system act as barriers that compromise not only adherence to treatment but also the establishment of an effective bond between the professional and the family unit. Objective: To evaluate the profile and frequency of pharmaceutical teleconsultations, as well as their contribution to the adherence, safety, and effectiveness of the treatment of the patients followed. Description: Retrospective, descriptive, and quantitative study with data from 01/2025 to 12/2025, obtained from the medical records of patients followed by the outpatient clinical pharmacy of ICr-HC-FMUSP via teleconsultation using an institutional cell phone, with the consent of the responsible party. The teleconsultations were conducted using the corporate cell phone provided by the Institution. Contact was made through a messaging application with prior authorization from the responsible party, with the aim of monitoring pharmacotherapeutic treatment. In 2025, 364 teleconsultations were recorded. Adherence monitoring predominated, with 207 (56.87%) records focused on monitoring compliance with the treatment plan established in consultation. Pharmaceutical guidance totaled 110 (30.22%) of the teleconsultations, most of which were motivated by doubts or adjustments in the method of use (30%), which includes instructions on dosage and administration. Monitoring of frequency and absenteeism due to failure to pick up medications at the pharmacy (20.91%) and substitutions of prescription items (15.45%), resulting from clinical or logistical needs, also showed a significant volume. In addition, treatment inclusions (9.10%) and management of adverse drug reactions (9.10%), fundamental for patient safety, were observed. Adherence failures (3.63%), health problems (3.63%), and prescription errors (3.63%) were recorded, preventing potential adverse events, as well as parallel administrative demands that impact patient treatment (4.55%). Complementarily, 42 (11.54%) assisted doses were administered, with direct supervision of drug administration, and 5 (1.37%) pharmaceutical education actions were carried out, aiming to increase patient autonomy. Conclusion: The study highlights that tele-guidance does not replace in-person care, but acts as a strategic complement, especially in the follow-up of chronic patients and in resolving specific demands. The results presented reinforce that this modality contributes to improving the quality of care, optimizing resources, and strengthening clinical pharmaceutical care. Adherence actions predominated (56.87%), highlighting the pharmacist's role in the effectiveness and prevention of drug-related problems. Tele-guidance allowed for monitoring, identification of failures, adverse events, and errors. Demands regarding use and absenteeism indicate mitigable weaknesses, reinforcing education, bonding, and patient safety.
