Resumo
Objetivos: A cirurgia bariátrica é um processo altamente complexo indicado para tratar a obesidade mórbida. No SUS, a cirurgia oferecida é realizada por laparotomia. Os objetivos do estudo foram: elaborar uma revisão rápida sobre a eficácia e análise econômica da cirurgia bariátrica para decisão de incorporação da técnica de videolaparoscopia no SUS. Realizar um levantamento sobre o número de hospitalizações e despesas relacionadas às cirurgias bariátricas realizadas no Brasil no período de 2008 a 2012. Métodos: Busca, análise e uso crítico da literatura científica baseada em Medline, Pesquisa Saúde e Rebrats (Rede Brasileira de Avaliação de Tecnologias em Saúde). Prioridade foi dada a estudos Brasileiros e estudos financiados pelo Departamento de Ciência e Tecnologia (DECIT) do Ministério da Saúde do Brasil. Aquisição de dados sobre despesas relacionadas à cirurgia bariátrica no banco de dados do Departamento de Informática do SUS (DATASUS). Resultados: Não foram observadas diferenças significativas entre os métodos laparotômico e laparoscópico em termos de mortalidade e perda de peso. No entanto, houve diferenças significativas na qualidade de vida do paciente e no tempo de internação. Estudos financiados pelo DECIT mostram que o custo da cirurgia varia de acordo com a via de acesso. O gasto médio para videolaparoscopia foi de US$12.448,00 em comparação com US$4.911,00 para a cirurgia convencional. Dados do DATASUS indicam que o número de hospitalizações para cirurgia bariátrica custeadas pelo SUS foi de 16.785. Neste período, observou-se um aumento de mais de 68%. O valor agregado das despesas neste campo foi de US$44.116.330,031 para esses anos. Conclusões: Os resultados mostram que o número de cirurgias bariátricas aumentou significativamente nos últimos anos. Portanto, é necessário realizar uma análise de custo-benefício do ponto de vista do SUS. Esta análise deve considerar as implicações para a qualidade de vida dos pacientes ao avaliar a cirurgia bariátrica por videolaparoscopia.

Objetives: Bariatric surgery is highly complex process indicated for treating morbid obesity. In the SUS the surgery provided is the laparotomical way. Objetives: of the study were: Elaboration of a rapid review about the effectiveness and economical analysis of the bariatric surgery Methods for the decision of incorporating the videolaparoscopy technique into SUS. Survey about the number of hospitalization and expenditures related to bariatric surgeries effectuated in Brazil in the period between 2008 and 2012. Methods: Search, analysis and critical scientific literature usage based on Medline, Pesquisa Saúde and Rebrats (The Brazilian Network for Health Technology Assessment). A priority was attributed to Brazilian studies and studies financed by Department of Science and Technology (DECIT) of the Ministry of Health of Brazil. Data acquisition of expenditures related to bariatric surgery in the database of the IT Department of SUS (DATASUS). Results: There have been no observable differences between laparotomical and laparoscopical Methods in such terms as mortality and weight loss. But there have been significant differences in the quality of life of the patient and internalization time. The studies financed by DECIT show that the cost of the surgery varies according to the access route. The average expenditure for videolaparoscopy was US$12,448.00 compared to US$4,911.00 for the conventional surgery. Data of DATASUS indicates that the number of hospitalization for bariatric surgery paid by SUS was 16.785. In this period an increase of more than 68% was observed. The aggregate value of the expenditure in this field was US$44,116,330.031 for these years. Conclusions: The Results show that the number of bariatric surgery has increased significantly in the last years. Therefore it's necessary to conduct a cost-benefit analysis in the standpoint of the SUS. This analysis should consider the implications to the quality of live of patients when evaluating videolaparoscopy bariatric surgery way.
