Objetivos: Isotretinoína, um medicamento usado para tratar acne severa, tem sido objeto de vários estudos para investigar a relação entre a terapia e distúrbios psiquiátricos. Ao mesmo tempo, a demanda por este item tem crescido no Sistema Único de Saúde (SUS), que incorporou o medicamento desde 2001 e seu uso é padronizado pelo Protocolo Clínico e Diretrizes Terapêuticas do SUS. O objetivo deste trabalho foi avaliar quantitativamente a demanda de isotretinoína no ano de 2011 e o impacto financeiro deste item no total distribuído no Componente Especializado da Assistência Farmacêutica (CEAF), programa de acesso a medicamentos do SUS. Métodos: Foram levantados dados de demanda e valor financeiro distribuído de isotretinoína 20 mg em 2011 no SIGAF (Sistema de Gestão da Assistência Farmacêutica do Estado de Minas Gerais) em relação ao montante total distribuído para todos os itens do CEAF. Resultados: Em 2011, a demanda por isotretinoína foi de 9.374 novos pacientes, o segundo colocado; hidróxido de ferro 100 mg, 3.111, e budesonida + formoterol 400/200 mcg ficaram em terceiro lugar com 2.177. O montante total distribuído foi equivalente a 8.715 milhões de dólares, 8% do total gasto com 196 medicamentos que compõem o CEAF. Conclusões: Isotretinoína foi o medicamento mais solicitado em 2011 no CEAF, três vezes maior que o segundo e quatro vezes maior que o terceiro. Ao mesmo tempo, os gastos com este medicamento são relevantes, considerando a porcentagem do total gasto com todos os medicamentos do CEAF. Isso representa 11,6% do total gasto no Estado de Minas Gerais com o tratamento de doenças cardiovasculares, as doenças crônicas mais prevalentes no Brasil segundo a OMS. Considerando os riscos conhecidos e a falta de estudos conclusivos sobre outros efeitos adversos da isotretinoína, os resultados destacam a necessidade de monitoramento mais próximo dos pacientes e controle deste medicamento, a fim de evitar o consumo desenfreado e irracional.

Objetives: Isotretinoin, medicine used to treat severe acne has been the subject of various studies to investigate the relationship between therapy and psychiatric disorders. At the same time, the demand has been growing this item in the National Health System (SUS) which incorporated the drug since 2001 and its use is standardized by the Clinical Protocol and Therapeutic Guidelines by SUS. The aim of work was to quantitatively evaluate the demand Isotretinoin of in the year 2011 and the financial impact of this item in the total distributed in Component Specialized Pharmaceutical Services –CEAF (program for access to medicines under SUS) Methods: Survey demand data and distributed financial value of Isotretinoin 20 mg of 2011 in SIGAF-(Computerized Management of the State Pharmaceutical Assistance Minas Gerais) in relation to the total amount distributed to all items of CEAF.Results: In 2011, the demand of the Isotretinoin was 9374 new patients, the second-placed; iron hydroxide 100 mg, 3111, budesonide + formoterol 400/200 mcg was placed third with 2177. The total amount distributed was equivalent to 8715 million dollars, 8% of total expended with 196 medicines that make up the CEAF. Conclusions: Isotretinoin was the most requested in 2011 in CEAF, three times higher than the second and 4 times higher than the third. At the same time, spending on this medicine is relevant, considering the percentage of the total spent on all CEAF’s medications. This represents 11.6% of the total amount spent in the State of Minas Gerais with the treatment of cardiovascular diseases, chronic diseases most prevalent in Brazil according to WHO. Considering the known risks and the lack of conclusive studies on other adverse effects of Isotretinoin, the Results highlight the need for closer monitoring of patients and control this medicine, in order to avoid the rampant consumption and irrational.
