Contexto e Introdução: A Comissão Nacional de Incorporação de Tecnologia (CONITEC) é responsável por gerenciar o processo de atualização das diretrizes e por desenvolver estratégias para identificar e reduzir barreiras à implementação das diretrizes produzidas no Sistema Único de Saúde (SUS) do Brasil. Dentre as estratégias disponíveis, a consulta pública online pode ser utilizada para identificar essas barreiras no início do processo de atualização das diretrizes. Objetivos/Meta: Descrever o método e as barreiras de implementação identificadas em oito diretrizes. Métodos: Oito diretrizes, a serem atualizadas - Hipertensão Arterial Pulmonar, Imunossupressão no Transplante Renal, Asma, Doença de Crohn, Osteoporose, Sobrecarga de Ferro e Transtorno Esquizoafetivo, foram submetidas à consulta pública online de setembro a outubro de 2016. Para identificar possíveis barreiras, foi formulada uma questão específica: "considerando sua realidade local, o que dificulta a implementação desta diretriz atualmente". Resultados e Discussão: Foram recebidas 305 contribuições: 59 pacientes, 81 profissionais de saúde, 64 especialistas, 24 empresas farmacêuticas, 12 sociedades médicas, 12 associações de pacientes e 53 outros stakeholders. Foram relatadas 261 barreiras de implementação, sendo as mais frequentes: (i) acesso a medicamentos, (ii) acesso a novas tecnologias não cobertas pelo sistema de saúde e (iii) dificuldades de acesso a serviços de saúde. Implicações para desenvolvedores/usuários de diretrizes: Identificar as barreiras existentes é um passo importante no processo de implementação das diretrizes e um indicador para o desenvolvimento ou atualização de recomendações mais viáveis. Conclusão A consulta pública parece ser uma ferramenta útil na atualização das diretrizes no SUS, permitindo identificar barreiras na implementação das recomendações existentes e priorizar futuras questões de pesquisa para atualizar as diretrizes.

Background & Introduction: National Committee for Technology Incorporation (CONITEC) is responsible for managing the process of updating the guidelines and for developing strategies for identifying and reducing barriers to the implementation of the guidelines produced in the Brazilian Public Health System (SUS). Among the available strategies, the online public consultation can be used to identify these barriers at the beginning of the process of updating the guidelines. Objectives/Goal: To Describe the method and implementation barriers identified in eight guidelines. Methods: Eight guidelines, to be updated - Pulmonary Arterial Hypertension, Immunosuppression in Renal Transplantation, Asthma, Crohn's Disease, Osteoporosis, Iron Overload and Schizoaffective Disorder, were submitted to the online public consultation from September to October 2016. To identify possible barriers a specific issue was formulated: "considering its local reality, which makes it difficult to implement this guideline currently". Results & Discussion: A total of 305 contributions were received: 59 patients, 81 health professionals, 64 specialists, 24 pharmaceutical companies, 12 medical societies, 12 patient associations and 53 other stakeholders. 261 implementation barriers have been reported, most frequently: (i) access to medicines (ii) access to new technologies not covered by the health system and (iii) difficulties in accessing health services. Implications for guideline developers/users: Identifying existing barriers is an important step in the implementation process of the guidelines and an indicator for the development or updating of more feasible recommendations. Conclusion The public consultation seems to be a useful tool in updating the guidelines in SUS, allowing to identify barriers in the implementation of existing recommendations and prioritize future research questions to update guidelines.
