Resumo
Introdução: A PAF é uma doença hereditária que afeta o sistema nervoso periférico. Em 2018 foi aprovado o Protocolo Clínico e Diretrizes Terapêuticas (PCDT) da PAF. O custeio baseado em atividades orientado pelo tempo (TDABC) é uma metodologia que permite que as organizações de saúde tenham uma visão mais precisa dos custos associados aos seus serviços. Ao usar o TDABC, as organizações podem identificar quais atividades são mais caras e, assim, encontrar maneiras de otimizar processos e reduzir custos sem comprometer a qualidade do atendimento ao paciente.Métodos: Este estudo descreve a implementação do TDABC no JAV RARAS, com o propósito de quanti-ficar os custos relacionados ao tratamento da PAF. O PCDT foi utilizado como protocolo base para aná-lise do TDABC. Os dados de custo foram obtidos por meio de entrevistas e levantamentos com dados administrativos, enfermagem e profissionais de saúde, nos centros participantes nas regiões Sudeste e Nordeste do Brasil. Foram estudados os custos diretos, envolvendo a identificação das atividades no manejo da PAF, a alocação dos recursos consumidos, a estimativa do custo por unidade de tempo, e a mensuração do tempo de atividade. Além disso, o estudo possibilitou a validação de protocolos que abrangem a jornada assistencial do paciente com doenças raras, contribuindo para uma compreensão mais precisa do custo anual da assistência ao paciente com PAF em âmbito nacional. Este estudo foi financiado pelo Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq) e pelo Ministé-rio da Saúde MS - CNPq/MS Nº 25/2019.Resultados: Em média, um paciente com PAF gasta anualmente R$198.853,14 em sua jornada de tra-tamento, sendo que a grande parte desse valor é destinado aos medicamentos. Os custos são recursos humanos (R$253,20), materiais (R$489,54), medicamentos (R$197.240,40) e exames (R$870,00). O valor total anual é dedicado exclusivamente ao tratamento da doença. Em relação à origem dos recursos, cerca de 99% são provenientes dos próprios centros de tratamento onde o paciente é acom-panhado, enquanto cerca de 1% é custeado pelo SUS em outros locais.Discussão e conclusões: Este estudo ressalta a importância de investigar os custos e o acesso a inter-venções terapêuticas no tratamento da PAF. Os medicamentos representam a maior parcela dos gastos anuais, exigindo uma compreensão mais aprofundada dos determinantes desses custos. Compreender os custos diretos em escala nacional é fundamental para uma alocação precisa de recursos e a sus-tentabilidade do sistema de saúde. A busca por alternativas terapêuticas mais eficazes pode reduzir os custos a longo prazo e melhorar os desfechos clínicos dos pacientes com PAF. Essas informações são cruciais para orientar decisões políticas, investimentos e avanços no cuidado de pacientes com PAF e outras doenças raras.

Abstract
Introduction: FAP is an inherited disease that affects the peripheral nervous system. In 2018, the FAP Clinical Protocol and Therapeutic Guidelines (PCDT) was approved. Time-driven activity-based costing (TDABC) is a methodology that allows healthcare organizations to have a more accurate view of the costs associated with their services. By using TDABC, organizations can identify which activities are most expensive and thus find ways to optimize processes and reduce costs without compromising the quality of patient care. Methods: This study describes the implementation of TDABC at JAV RARAS, with the purpose of quantifying the costs related to the treatment of FAP. PCDT was used as the base protocol for TDABC analysis. Cost data were obtained through interviews and surveys with administrative data, nursing and healthcare professionals, at participating centers in the Southeast and Northeast regions of Brazil. Direct costs were studied, involving the identification of activities in the management of FAP, the allocation of resources consumed, the estimation of the cost per unit of time, and the measurement of activity time. In addition, the study allowed the validation of protocols that cover the care journey of patients with rare diseases, contributing to a more accurate understanding of the annual cost of care for patients with FAP at the national level. This study was funded by the National Council for Scientific and Technological Development (CNPq) and by the Ministry of Health MS - CNPq/MS No. 25/2019. Results: On average, a patient with FAP spends R$198,853.14 annually on their treatment journey, with a large part of this amount going to medications. The costs are human resources (R$253.20), materials (R$489.54), medications (R$197,240.40), and exams (R$870.00). The total annual amount is dedicated exclusively to the treatment of the disease. Regarding the origin of the resources, approximately 99% comes from the treatment centers where the patient is followed, while approximately 1% is funded by the SUS in other locations. Discussion and conclusions: This study highlights the importance of investigating the costs and access to therapeutic interventions in the treatment of FAP. Medications represent the largest share of annual expenditures, requiring a more in-depth understanding of the determinants of these costs. Understanding direct costs on a national scale is essential for an accurate allocation of resources and the sustainability of the health system. The search for more effective therapeutic alternatives can reduce long-term costs and improve clinical outcomes for patients with FAP. This information is crucial to guide policy decisions, investments, and advances in the care of patients with FAP and other rare diseases.
