Resumo
Introdução: A Alergia à Proteína do Leite de Vaca (APLV) se refere às reações imunologicamente mediadas, podendo ser IgE mediada, não IgE mediada ou mista. Essa condição pode afetar os sistemas digestivo, respiratório, cutâneo, e desencadear reações sistêmicas graves, como a anafilaxia. O tratamento é a exclusão da proteína alergênica da dieta. A prevalência de APLV é em torno de 2,5% em crianças e em 80-90% desenvolvem tolerância até o 3º ano de vida3. Um Protocolo Clínico e Diretrizes Terapêuticas (PCDT) Estadual de 2007 normatiza a dispensação de fórmulas nutricionais para crianças com APLV até 24 meses, porém em agosto/2024 não há políticas nacionais para essa população no Sistema Único de Saúde (SUS). Objetivo: Revisar o histórico de avaliação de tecnologias para APLV em âmbito nacional no SUS, apresentar o cenário de implementação de um PCDT Estadual e desafios dessa política pública. Material e Método: Foram consultados documentos oficiais do Ministério da Saúde (MS) de 2012 a 2024 sobre o tema. Levantou-se informações de dispensação de fórmulas nutricionais para APLV, em sistema próprio da Secretaria de Estado da Saúde de São Paulo (SES/SP), nos períodos de maio/2021 e abril de 2022 e de maio/2023 e abril/2024. Resultados: Em 2012, a Secretaria de Atenção à Saúde/MS submeteu as fórmulas para APLV na CONITEC, que foi encerrada a pedido do demandante em 2015. Em 2017, houve nova submissão, que resultou na incorporação das fórmulas nutricionais para crianças até 24 meses com APLV no SUS (Portaria nº 67/2018)4. Em 2021, foi divulgada a Consulta Pública (CP) para o Teste de Provocação Oral (TPO)4, com recomendação preliminar favorável para incorporação no SUS4 e em 2022 foi incorporado no SUS para diagnóstico e monitoramento destes pacientes. O PCDT foi desenvolvido pelo MS e a CP finalizada em maio/2024. Até julho/2024 a versão final deste PCDT não havia sido publicada pelo MS4. Durante este período, a SES manteve o PCDT Estadual em funcionamento. Entre maio/2021 e abril/2022, 29.267 pacientes foram atendidos, sendo 57% com a dispensação de fórmulas de aminoácidos livres, 36% extensamente hidrolisada e 8% de proteína isolada de soja. Em comparação, entre maio/2023 e abril/2024, 37.551 pacientes foram atendidos (28% a mais), com proporções de 58%, 35% e 7%, respectivamente, para as mesmas fórmulas. Conclusões: Apesar da incorporação de tecnologias para APLV no SUS, sua oferta não se concretizou. Conforme legislação, a dispensação das fórmulas nutricionais deveria ter sido iniciada até maio/2019 (atraso em 1.895 dias em julho/2024) e o TPO deveria estar acessível à população desde setembro/2022 (atraso em 674 dias em julho/2024). Considerando que o PCDT ainda não foi publicado pelo MS, não há previsão de disponibilização das tecnologias no SUS. Esta lacuna assistencial tem sido suprida por iniciativas de gestores subnacionais, como a SES/SP, com financiamento próprio. Problemas de implementação de políticas públicas nacionais para APLV contribuem para a persistência de uma lacuna assistencial.

Abstract
Introduction: Cow's Milk Protein Allergy (CMPA) refers to immunologically mediated reactions, which may be IgE-mediated, non-IgE-mediated, or mixed. This condition can affect the digestive, respiratory, and cutaneous systems, and trigger severe systemic reactions, such as anaphylaxis. Treatment is the exclusion of the allergenic protein from the diet. The prevalence of CMPA is around 2.5% in children, and 80-90% develop tolerance by the 3rd year of life3. A 2007 State Clinical Protocol and Therapeutic Guidelines (PCDT) standardizes the dispensing of nutritional formulas for children with CMPA up to 24 months of age, but in August/2024 there are no national policies for this population in the Unified Health System (SUS). Objective: To review the history of technology assessment for CMPA at the national level in the SUS, present the scenario of implementation of a State PCDT and challenges of this public policy. Material and Method: Official documents from the Ministry of Health (MS) on the subject from 2012 to 2024 were consulted. Information on the dispensing of nutritional formulas for CMPA was collected in the system of the São Paulo State Health Department (SES/SP) in the periods May/2021 and April/2022 and May/2023 and April/2024. Results: In 2012, the Health Care Department/MS submitted the formulas for CMPA to CONITEC, which was closed at the request of the plaintiff in 2015. In 2017, there was a new submission, which resulted in the incorporation of nutritional formulas for children up to 24 months with CMPA in the SUS (Ordinance No. 67/2018)4. In 2021, the Public Consultation (CP) for the Oral Provocation Test (OPT)4 was published, with a preliminary favorable recommendation for incorporation into the SUS4 and in 2022 it was incorporated into the SUS for the diagnosis and monitoring of these patients. The PCDT was developed by the MS and the CP was finalized in May/2024. Until July/2024, the final version of this PCDT had not been published by the MS4. During this period, the SES kept the State PCDT in operation. Between May/2021 and April/2022, 29,267 patients were treated, 57% with the dispensing of free amino acid formulas, 36% extensively hydrolyzed and 8% isolated soy protein. In comparison, between May/2023 and April/2024, 37,551 patients were treated (28% more), with proportions of 58%, 35% and 7%, respectively, for the same formulas. Conclusions: Despite the incorporation of technologies for CMPA in the SUS, their provision has not materialized. According to the legislation, the dispensing of nutritional formulas should have started by May/2019 (delay of 1,895 days in July/2024) and the TPO should have been accessible to the population since September/2022 (delay of 674 days in July/2024). Considering that the PCDT has not yet been published by the MS, there is no forecast for the availability of the technologies in the SUS. This care gap has been filled by initiatives of subnational managers, such as SES/SP, with their own funding. Problems in the implementation of national public policies for CMPA contribute to the persistence of a care gap.
