Resumo
Objetivos: Em 1996, a OPAS / OMS e a UNICEF desenvolveram a estratégia IMCI com o objetivo de melhorar o desempenho dos profissionais de saúde, fortalecer o apoio ao sistema de saúde e aprimorar o conhecimento sobre as melhores práticas de cuidado infantil em casa e na comunidade. O objetivo deste estudo é avaliar a estratégia IMCI através das percepções dos gestores de saúde. Métodos: Desenho qualitativo usando entrevista semiestruturada. População do Estudo: pessoas-chave, gestores de organizações de saúde governamentais e não governamentais que participaram da estratégia IMCI no Estado do Ceará (Nordeste do Brasil) e no Peru. As entrevistas foram realizadas durante maio-junho de 2011, gravadas e posteriormente transcritas. Para a análise, foi utilizado um método de triangulação com outro pesquisador, revisando a literatura e outros documentos. Resultados: Foi observada uma importante redução na taxa de mortalidade infantil no Ceará (de 32 por 1000 nascidos vivos em 1997 para 15,6 em 2009) e no Peru (de 43 por 1000 nascidos vivos em 1996 para 17 em 2008) devido à diminuição da mortalidade pós-neonatal por diarreia e pneumonia. Em ambos os locais, no início houve um treinamento em larga escala para a equipe de saúde, médicos e enfermeiros, e menor para os agentes comunitários de saúde. A evolução da estratégia foi diferente. No Ceará, houve uma diminuição do interesse e falta de apoio por parte dos governos. No Peru, o governo adotou a estratégia; além disso, houve uma maior incorporação ao ensino universitário, aprendizado a distância e a adição de novos conteúdos (perinatal / neonatal, asma e síndrome bronco-obstrutiva, desenvolvimento infantil, saúde bucal, abuso, violência e acidentes, diabetes e obesidade). Conclusões: A estratégia IMCI foi desenvolvida de forma diferente nos países estudados. Essas informações podem ser usadas para avaliar a estratégia e a participação dos diferentes setores responsáveis pela saúde infantil.

Abstract
Objetives: In 1996 PAHO / WHO and UNICEF developed the IMCI strategy with the aim of improving the health workers’ performance, strengthening the health system support and improving the knowledge about the children care best practices at home and in the community. The Objetive of this study is to evaluate the IMCI strategy through the health managers’ perceptions. Methods: Qualitative design using semi-structured interview. Population Study: key persons, governmental and nongovernmental health organizations managers who participated in the IMCI strategy in the State of Ceará (Northeast Brazil) and in Peru. The interviews were conducted during May-June 2011. They were tape recorded and transcribed later. For the analysis, a triangulation method was carried out with another researcher, reviewing the literature and other documents. Results: An important reduction in infant mortality rate was observed in Ceará (from 32 per 1000 live births in 1997 to 15.6 in 2009) and in Peru (from 43 per 1000 live births in 1996 to 17 in 2008) by the decrease in post-neonatal mortality due to diarrhea and pneumonia. In both places at the beginning there was a large-scale training for the health staff, doctors and nurses, and less for the community health agents. The evolution of the strategy has been different. In Ceará there was a decrease of the interest and a lack of support from governments. In Peru, the government adopted the strategy; likewise there was a greater incorporation into the university teaching, distance learning and the addition incorporation of new content (perinatal / neonatal, asthma and broncho-obstructive syndrome, child development, oral health, abuse, violence and accidents, diabetes and obesity). Conclusions: The IMCI strategy has been developed differently in the studied countries. This information can be used to evaluate the strategy and the participation of the different sectors responsible for the child health.
