Resumo
Objetivos: O protocolo Brasileiro recomenda que o tratamento de primeira linha para pacientes com hepatite C crônica (HCC) e genótipo 2 ou 3 seja interferon alfa (IFN) mais ribavirina por 24 semanas. Para aqueles que não respondem a esse tratamento, é recomendado o uso de peginterferon alfa (PEG) mais ribavirina por 48 semanas. Nosso objetivo foi comparar o custo e a eficácia do tratamento de primeira linha com peginterferon com o protocolo Brasileiro atual. Métodos: População-alvo: Pacientes com HCC e genótipo 2 ou 3 no Brasil. Intervenções: PEG-SEC: interferon alfa (IFN) mais ribavirina (RBV) por 24 semanas para pacientes com genótipo 2/3; para não respondedores, subsequente peginterferon (PEG) mais RBV por 48 semanas; PEG-FIRST: PEG+RBV por 48 semanas para todos os pacientes. Tipo de estudo: Análise de custo-efetividade. Fontes de dados: Dados de eficácia de uma meta-análise conduzida com estudos Brasileiros. Custo do tratamento com antivirais, medicamentos secundários, testes diagnósticos, consultas ambulatoriais com médicos e outros profissionais, hospitalizações, cuidados de enfermagem e farmacêuticos de um estudo de micro-custeio convertido para USD 2010. Perspectiva: Sistema de saúde. Medida de resultado: Resposta Viral Sustentada (SVR), custos diretos e razão de custo-efetividade incremental (ICER). Resultados: Com uma taxa de SVR de 76,6% e custos de USD 6.943, PEG-SEC foi mais eficaz e menos custoso do que PEG-FIRST (SVR: 73,2%, custos: USD 11.297). Análises de sensibilidade: Para que PEG-SEC continue dominante, a proporção de pacientes submetidos ao tratamento de segunda linha com PEG+RBV deve ser >88%. Se apenas 0,5% dos pacientes forem submetidos ao tratamento de segunda linha com PEG+RBV, o ICER de PEG-FIRST é de USD 71.529 por SVR adicional. Conclusões: No contexto Brasileiro, IFN para genótipo 2 ou 3 como tratamento de primeira linha, e PEG+RBV para aqueles que não alcançam SVR é mais eficaz e menos custoso do que PEG+RBV como tratamento de primeira linha.

Abstract
Objetives: The Brazilian protocol recommends that the first line treatment for patients with chronic hepatitis C (CHC) and genotype 2 or 3 is interferon alfa (IFN) plus ribavirin for 24 weeks. For those that do not respond to this treatment the use of peginterferon alfa (PEG) plus ribavirin for 48 weeks is recommended. Our Objetive was to compare the cost and effectiveness of first line treatment of genotype 2 or 3 with peginterferon with the current Brazilian protocol. Methods: Target Population: CHC patients with genotype 2 or 3 in Brazil. Interventions: PEG-SEC: interferon alfa (IFN) plus ribavirin (RBV) for 24 weeks for patients with genotype 2/3; for nonresponders subsequently peginterferon (PEG) plus RBV for 48 weeks; PEG-FIRST: PEG+RBV for 48 weeks for all patients. Study Type: Cost-effectiveness analysis. Data Sources: Effectiveness data from a meta-analysis conducted with Brazilian studies. Treatment cost for antiviral drugs, secondary drugs, diagnostic tests, outpatient visits to physicians and other professionals, hospitalizations, nurse and pharmaceutical care from a micro-costing study converted to 2010 USD. Perspective: Health care system. Outcome Measure: Sustained Viral Response (SVR), direct costs and incremental cost effectiveness ratio (ICER). Results: With a SVR rate of 76.6% and costs of USD 6,943, PEG-SEC was more effective and less costly than PEG-FIRST (SVR: 73.2%, costs: USD 11,297). Sensitivity analyses: For PEG-SEC to remain dominant, the proportion of patients undergoing second line treatment with PEG+RBV must be >88%. If only 0.5% of patients undergo second line treatment with PEG+RBV, the ICER of PEG-FIRST is USD 71,529 per additional SVR. Conclusions: In the Brazilian context, IFN for genotype 2 or 3 as first line, and PEG+RBV for those who fail to achieve SVR is more effective and less costly than PEG+RBV as first line treatment.
