Objetivos: Estimar o impacto orçamentário do PP-LAI no tratamento da esquizofrenia crônica no Brasil sob a perspectiva do pagador público, especificamente para pacientes com problemas de adesão. Métodos: O modelo de impacto orçamentário (BIM) foi utilizado para estimar a população elegível e projetar os investimentos para os primeiros 5 anos (2012-2016). Foi adotada uma abordagem baseada em taxas de prevalência e adesão reportadas em estudos anteriores para definir a população elegível. Foram simulados dois cenários: 1) o cenário atual, no qual pacientes esquizofrênicos com problemas de adesão recebem medicamentos atípicos já fornecidos pelo Sistema Único de Saúde (SUS) (olanzapina, quetiapina e ziprasidona), e 2) o cenário proposto, no qual pacientes com comportamentos de não-adesão à terapia oral atípica podem receber o PP-LAI. Os custos finais foram calculados considerando os custos de aquisição dos medicamentos e o consumo de recursos de saúde relacionados ao acompanhamento ambulatorial, hospitalizações e eventos adversos. Resultados: O BIM estimou a população elegível para o PP-LAI variando de 2.207 pacientes (2012) a 2.271 pacientes (2016), aproximadamente 0,3% da população projetada de esquizofrenia atendendo aos serviços do SUS. Com todos os medicamentos isentos de impostos, os custos de aquisição foram: PP-LAI 5.566 BRL (1º ano) e 4.963 BRL (anos subsequentes); olanzapina 3.850 BRL; quetiapina 5.416 BRL; ziprasidona 1.474 BRL. O modelo estimou que as despesas atuais do SUS seriam de 287 milhões BRL em 2012, alcançando 296 milhões BRL em 2016. Se o PP-LAI for incluído na lista de reembolso, o impacto orçamentário anual em 2012 seria de 2,4 milhões BRL (+0,83%) e 1,1 milhões BRL em 2016 (+0,37%). O impacto orçamentário acumulado chegaria a 6.658.573 BRL em 5 anos. Conclusões: O PP-LAI é uma opção terapêutica eficaz para pacientes esquizofrênicos, particularmente aqueles que se beneficiariam de medicamentos antipsicóticos de liberação prolongada. Neste BIM, os investimentos para incluir o PP-LAI nas listas de reembolso do SUS foram estimados em 6,7 milhões BRL em cinco anos (um incremento de 0,46% no orçamento atual).

Abstract
Objetives: To estimate the budget impact of PP-LAI in treating chronic schizophrenia in Brazil from the perspective of the public payer, specifically for patients with adherence issues. Methods: The budget impact model (BIM) was used to estimate eligible population and to project investments for the first 5y (2012-2016). An approach based on prevalence and adherence rates reported by previous studies was employed to define the eligible population. Two scenarios were simulated: 1) the current setting, in which schizophrenic patients with adherence issues receive atypical drugs already provided by the Brazilian Public Healthcare System (BPHS) (olanzapine, quetiapine and ziprasidone), and 2) the proposed setting, in which patients presenting non-adherence behaviors on oral atypical therapy can receive PP-LAI. Final costs were calculated considering drug acquisition costs and health resource consumption related to outpatient follow-up, hospitalizations and adverse events. Results: The BIM estimated the population eligible for PP-LAI ranging from 2,207 patients (2012) to 2,271 patients (2016), approximately 0.3% of the projected schizophrenia population attending BPHS services. With all drugs tax exempted, the acquisition costs were: PP-LAI 5,566BRL (1st year) and 4,963BRL (subsequent years); olanzapine 3,850BRL; quetiapine 5,416BRL; ziprasidone 1,474BRL. The model estimated the current BPHS expenses at 287 million BRL in 2012, reaching 296 million BRL in 2016. If PP-LAI is included in the reimbursement list, the yearly budget impact in 2012 would be 2.4 million (+0.83%) and 1.1 million in 2016 (+0.37%). The cumulative budget impact would reach 6,658,573BRL in 5 years. Conclusions: PP-LAI is an effective therapeutic option for schizophrenic patients, particularly those who would benefit from LAI antipsychotic drugs. In this BIM, the investments to include PP-LAI on BPHS reimbursement lists were estimated to be BRL 6.7 million in five years (an increment of 0.46% in the current budget).
