Objetivos: Novos tratamentos em oncologia frequentemente implicam em custos mais altos. Historicamente, há uma falta de dados estatísticos sobre tratamentos de câncer no SUS no Brasil. Custos mais altos combinados com a falta de informação podem resultar em desperdício de recursos. Apresentamos aqui dados do mundo real (DWR) sobre tratamento de MM coletados de uma base de dados dedicada a tratamentos de câncer, Evidencias® (www\.evidencias.com.br). Métodos: Entre novembro de 2007 e outubro de 2009, recuperamos todos os pacientes com MM registrados no Evidencias®. Dados antropométricos, estadiamento, tipos de tratamento, duração e custo da quimioterapia e terapias adjuntas (TA), como antieméticos, eritropoietina, fatores estimulantes de colônias e bifosfonatos foram extraídos. Resultados: Identificamos 98 pacientes (53% mulheres e 47% homens). A área de superfície corporal (ASC) média foi de 1,73 m². O estadiamento foi estratificado da seguinte forma: 21% Estágio I, 8% Estágio II e 54% Estágio III. Em 17% dos casos, a informação de estadiamento estava indisponível. O custo total dos medicamentos foi de US$1.709.404,50. Medicamentos quimioterápicos representaram US$1.214.275,03 (71%), dos quais 98% (US$1.192.849,98) foram devido ao uso de Bortezomibe e 29% (US$495.129,46) a TA. O Bortezomibe esteve presente em 57% dos tratamentos, seja isolado (42%) ou combinado com outras quimioterapias (15%). Ciclofosfamida foi usada em 20% dos tratamentos, Doxorrubicina em 18% e Melphalan em 5%. A duração média do tratamento foi de 3,9 ciclos de quimioterapia. Conclusões: Dados do mundo real, embora sejam uma ferramenta fundamental para orientar os prestadores de cuidados de saúde na alocação correta de recursos, ainda são raros no Brasil e na América Latina. O Bortezomibe é usado em 57% dos tratamentos de MM, mas representa 98% dos custos com quimioterapia.

Higher costs combined to lack of information may result in waste of resources. We present here real world data (RWD) on MM treatment collected from a dedicated database of cancer treatments Evidencias® (www\.evidencias.com.br). Methods: Between November of 2007 and October 2009 we retrieved all patients with MM registered on Evidencias®. Anthropometric data, staging, types of treatment, duration, and cost of chemotherapy and adjunctive therapies (AT) such as antiemetics, erythropoietin, colony stimulating factors and bisphosphonates were extracted. Results: We identified 98 patients (53% women and 47% men). The mean body surface area (BSA) was1.73 m2. Staging was stratified as follows: 21% Stage I, 8% stage II and 54% stage III. In 17% of the cases stage information was unavailable. Total cost of drugs was US$1,709,404.50. Chemotherapy drugs represented US$1,214,275.03 (71%) of which 98% (US$1,192,849.98) were due to the use of Bortezomib and 29% (US$495,129.46) to AT. Bortezomib was present in 57% of the treatments either isolated (42%) or combined to other chemotherapy (15%). Cyclophosphamide was used in 20% of the treatments, Doxorubicin in 18% and Melphalan in 5%. Mean duration of treatment was 3.9 cycles of chemotherapy. Conclusions: Real world data although a fundamental tool to guide health care providers in the correct allocation of resources, is still rare in Brazil e Latin America. Bortezomib is used in 57% of MM treatments but represents 98% of chemotherapy costs.
