Objetivos: O sorafenibe melhora a sobrevivência em AHCC (Llovet, NEJM 2008) e tem sido considerado custo-efetivo no Canadá (Muszbek, Curr Med Res Opin 2008), quando comparado com BSC. Na prática clínica, no entanto, pacientes com AHCC sem acesso ao sorafenibe são frequentemente tratados com outros agentes sistêmicos, nenhum dos quais é capaz de melhorar o desfecho. O objetivo deste estudo foi avaliar a custo-efetividade de sorafenibe+BSC vs BSC sozinho no Brasil, sob a perspectiva do sistema privado de saúde. Métodos: Um modelo de Markov foi desenvolvido para projetar a sobrevivência e os custos vitalícios para ambas as intervenções, usando dados das curvas Kaplan-Meier de TTP e OS do estudo SHARP em uma distribuição lognormal e de um painel ad hoc com oncologistas clínicos, hepatologistas e cirurgiões hepáticos Brasileiros. A eficácia do tratamento foi medida em anos de vida ganhos (LYG). A utilização de recursos incluiu medicamento, administração, consultas médicas, monitoramento e eventos adversos. Custos (em R$, com R$ 1,00 \~ US$ 0,58) e benefícios de sobrevivência foram descontados anualmente em 5%. Análises de sensibilidade univariadas e probabilísticas foram realizadas. Resultados: Os custos vitalícios por paciente em R$ (US$) foram 103.210 (58.977) e 10.435 (5.963) para sorafenibe+BSC e BSC sozinho, respectivamente. O custo do medicamento sorafenibe representou cerca de 80% dos custos de tratamento. O benefício incremental de sobrevivência com sorafenibe+BSC foi de 0,49 anos de vida. A razão de custo-efetividade incremental de sorafenibe+BSC vs BSC sozinho foi de R$ 189.402 (US$ 108.230) por LYG. Conclusões: A adição de sorafenibe ao BSC é a única intervenção que demonstrou melhorar a sobrevivência em AHCC e os resultados de custo-efetividade refletem o aumento do custo de um tratamento ativo em comparação com uma alternativa de baixo custo e ineficaz.

In clinical practice, however, patients with AHCC with no access to sorafenib are often treated with other systemic agents, none of which are able to improve the outcome. The Objetive of this study was to evaluate the cost-effectiveness of sorafenib+BSC vs BSC alone in Brazil, from the perspective of the private health-care system. Methods: A Markov model was developed to project the lifetime survival and costs for both interventions using data from the TTP and OS Kaplan-Meier curves from SHARP trial in a lognormal distribution and from an ad hoc panel with Brazilian medical oncologists, hepatologists, and liver surgeons. Treatment effectiveness was measured in life-years gained (LYG). Resource utilization included drug, administration, physician visits, monitoring, and adverse events. Costs (in R$, with R$ 1.00 \~ US$ 0.58) and survival benefits were discounted annually at 5%. Univariate and probabilistic sensitivity analyses were conducted. Results: Lifetime per-patient costs in R$ (US$) were 103,210 (58,977) and 10,435 (5,963) for sorafenib+BSC and BSC alone, respectively. Sorafenib drug cost accounted for nearly 80% of treatment costs. The incremental survival benefit with sorafenib+BSC was 0.49 life-years. The incremental cost-effectiveness ratio of sorafenib+BSC vs BSC alone was R$ 189,402 (US$ 108,230) per LYG. Conclusions: The addition of sorafenib to BSC is the only intervention that has been found to improve survival in AHCC and the cost-effectiveness Results reflects the increased cost of an active treatment when compared with a low cost and ineffective alternative.
