Resumo
Contexto e objetivos: A imagem metabólica melhora a resolutividade dos programas de câncer e não estava disponível na saúde pública do Brasil. Pacientes com linfoma de Hodgkin (LH) recém-diagnosticado foram matriculados consecutivamente em dois centros, um no Brasil e outro na Itália, para este ensaio prospectivo que avaliou a relação custo-efetividade da tomografia por emissão de pósitrons com fluorodeoxiglicose marcada com flúor-18 (FDG-PET) no estadiamento inicial de pacientes com LH. Métodos: Todos os 210 pacientes (120 brasileiros e 90 italianos) foram estadiados com métodos convencionais de estadiamento clínico (CCS), incluindo tomografia computadorizada (TC), biópsia de medula óssea (BMB) e testes laboratoriais. Os pacientes também foram submetidos ao estadiamento metabólico (MS) com tomografia por emissão de pósitrons de corpo inteiro com FDG antes do início do tratamento. Um padrão de referência para estadiamento foi determinado com todos os procedimentos de estadiamento, exame histológico e exames de acompanhamento. A precisão do CCS foi comparada com o MS. Os custos locais dos procedimentos e testes foram avaliados. A razão de custo-efetividade incremental (ICER) foi calculada para ambas as estratégias. Resultados: Nos 210 pacientes com LH, dois terços dos casos brasileiros eram avançados (estágios III e IV foram 77 (64,2%) contra 32 (35,5%) entre os italianos). A sensibilidade para o estadiamento inicial do FDG-PET foi maior do que a da TC e da BMB no estadiamento inicial (97,9% contra 87,3%; P Conclusões: Atualmente, os linfomas brasileiros são detectados mais tarde do que na Itália. O FDG-PET é mais preciso do que a TC e a BMB no estadiamento do LH. Dadas as probabilidades observadas, o FDG-PET é altamente custo-efetivo no programa de saúde pública do Brasil. Embora limitado a um centro em cada país, esses resultados foram corroborados por outros estudos. Impacto da ATS: O plenário da Comissão Nacional de Incorporação de Tecnologias do Sistema de Saúde Pública, CONITEC / Ministério da Saúde do Brasil, endossou esses resultados e incluiu o PET/CT. As barreiras para a detecção precoce de linfomas foram priorizadas para financiamento de estudos. Além disso, essa colaboração formou a base para novos estudos e compartilhamento de dados internacionais.

Abstract
Background and objectives: Metabolic imaging improves cancer program resolutivity and was not available in public healthcare Brazil. Patients with newly diagnosed Hodgkin’s lymphoma (HL) were consecutively enrolled in two centers, one in each Brazil and Italy, for this prospective trial to evaluate the costeffectiveness of fluorine-18 (18F)-fluoro-2-deoxyD-glucose-positron emission tomography (FDGPET) scan in initial staging of patients with HL. Methods: All 210 patients (120 Brazilian and 90 Italian) were staged with conventional clinical staging (CCS) methods, including computed tomography (CT), bone marrow biopsy (BMB), and laboratory tests. Patients were also submitted to metabolic staging (MS) with whole-body FDG-PET scan before the beginning of treatment. A standard of reference for staging was determined with all staging procedures, histologic examination, and follow-up examinations. The accuracy of the CCS was compared with the MS. Local unit costs of procedures and tests were evaluated. Incremental cost-effectiveness ratio (ICER) was calculated for both strategies. Results: In the 210 patients with HL[1], two thirds of the Brazilian cases were advanced (stages III and IV were 77 (64.2%) vs. 32 (35.5%) amid the Italians). The sensitivity for initial staging of FDG-PET was higher than that of CT and BMB in initial staging #HTAiTokyo2016 Posters 159 (97.9% vs. 87.3%; P Conclusions: Currently, Brazilian lymphomas are detected latter than in Italy. FDG-PET is more accurate than CT and BMB in HL staging. Given observed probabilities, FDG-PET is highly cost-effective in the public healthcare program in Brazil. HTA Impact: The plenary of the Public Healthcare System Technology Incorporation National Commission, CONITEC/ Brazilian Health Ministry, endorsed these results and listed the PET/CT.
