Contexto e objetivos: A artrite reumatoide (AR) é uma doença inflamatória sistêmica, crônica e progressiva que afeta a membrana sinovial das articulações, podendo levar à destruição óssea e da cartilagem. O tratamento da AR inclui anti-inflamatórios não esteroides, corticoides e fármacos antirreumáticos modificadores da doença sintéticos (sDMARD) e biológicos (bDMARD). Os bDMARD demonstraram eficácia no tratamento da artrite reumatoide e são considerados onerosos. Consequentemente, o estudo avalia a eficácia dos bDMARD na prática clínica em uma coorte de pacientes com artrite reumatoide no Sistema Único de Saúde (SUS) do Brasil. Métodos: Foram incluídos indivíduos com diagnóstico de artrite reumatoide tratados com agentes anti-TNFα, adalimumabe e etanercepte, em um estudo de coorte prospectivo aberto. Para avaliar a eficácia dos agentes anti-TNFα, foi aplicado o Índice de Atividade da Doença Clínica (CDAI), comparando os resultados na linha de base e após 6 e 12 meses de acompanhamento. O fármaco foi considerado eficaz quando o paciente alcançou remissão ou baixa atividade da doença e não eficaz quando em atividade moderada ou alta. Resultados: Cento e trinta e sete pacientes completaram um ano de acompanhamento. O bDMARD mais utilizado foi o adalimumabe (75,9%), enquanto o etanercepte foi utilizado por 24,1%. Ambos os medicamentos, adalimumabe e etanercepte, reduziram significativamente a atividade da doença, medida pelo CDAI, durante um ano de acompanhamento (p < 0,001). Conclusões: Neste estudo de utilização de medicamentos em condições reais, metade dos pacientes com AR alcançou a meta de remissão ou baixa atividade da doença com o uso de adalimumabe e etanercepte, demonstrando que os bDMARD são alternativas viáveis para o tratamento da AR. Os pacientes restantes devem ter suas opções terapêuticas revisadas. Em vista do alto custo dos bDMARD para o SUS e, consequentemente, para a sociedade, em comparação com os sDMARD, um monitoramento farmacoterapêutico contínuo deve ser realizado por uma equipe multidisciplinar. Isso poderia alcançar melhores resultados, assegurando a qualidade do uso dos bDMARD, e será objeto de futuras pesquisas.

Abstract
Background and objectives: Rheumatoid arthritis (RA) is a systemic, chronic and progressive inflammatory disease which affects the synovial membrane of joints, and which may lead to bone and cartilage destruction. Treatment of RA includes non-steroidal anti-inflammatory, corticoids and synthetic (sDMARD) and biological (bDMARD) disease-modifying antirheumatic drugs. bDMARD has demonstrated efficacy for the treatment of rheumatoid arthritis and are 284 Posters #HTAiTokyo2016 considered costly. Consequently, the study evaluates the effectiveness of bDMARD in clinical practice on a cohort of patients with rheumatoid arthritis in the Public Health System (SUS), Brazil. Methods: Individuals with a diagnosis of rheumatoid arthritis treated with anti-TNF? agents, adalimumab and etanercept, were included in the open prospective cohort study. To assess the effectiveness of the TNF? agents, the Clinical Disease Activity Index (CDAI) was applied, comparing results at baseline and after 6 and 12 months of follow-up. The drug was considered effective when the patient achieved remission or low disease activity and not effective when in moderate or high disease activity. Results: One hundred and thirty-seven patients completed one year of follow-up. The most widely used bDMARD was adalimumab (75.9%), with etanercept used by 24.1%. The remaining patients should have their therapeutic options reviewed. In view of the high cost of the bDMARD to SUS and, consequently, to society, versus sDMARD continuous pharmacotherapeutic monitoring should be performed by a multidisciplinary team. This could achieve better results, assuring the quality of use of the bDMARD, and will be subject of further research.
