Introdução: A terapia utilizando tecnologias moleculares direcionadas (MTT) e anticorpos monoclonais (MA) são exemplos de novas tecnologias terapêuticas, em busca de maior eficácia clínica e redução de efeitos adversos no combate a doenças altamente frequentes. Geralmente, novas tecnologias impactam em altos custos. OBJETIVOS: Avaliar o impacto financeiro gerado pelo uso de terapias com MTT e MA no Hospital de Clínicas de Porto Alegre, Brasil. Métodos: Foram analisados os 60 itens de medicamentos com maiores gastos monetários dos últimos 12 meses. Dentre eles, foram identificados os medicamentos que se enquadravam nas categorias em estudo e que foram regularmente utilizados. Os gastos mensais com cada item foram tabulados e comparados com os gastos totais em medicamentos, a fim de calcular o impacto orçamentário. Os principais grupos de doenças tratadas com cada agente foram analisados. Resultados: Dois agentes de MTT (gefitinibe e infliximabe) e três MA (rituximabe, basiliximabe e abciximabe) foram identificados. Os itens de maior gasto, respectivamente, por ano, foram o medicamento oncológico gefitinibe (US$ 72.228,80), seguido pelos imunossupressores basiliximabe (US$ 97.138,50) e infliximabe (US$ 77.534,90), o oncológico rituximabe (US$ 72.228,80) e o inibidor da agregação plaquetária abciximabe (US$ 54.090,10), correspondendo, respectivamente, a 10,4%, 9,3%, 7,5%, 7,0% e 5,3% do gasto total com medicamentos por ano (US$ 13.403.300). Trastuzumabe, bortezomibe e imatinibe foram frequentemente utilizados, mas fornecidos diretamente pelo sistema público, de forma que não impactaram diretamente na gestão orçamentária do hospital. Conclusões: As MTT e MA têm um importante impacto orçamentário e são principalmente utilizadas para tratar alguns tipos de câncer, doenças cardiovasculares e terapias imunossupressoras. Esses aspectos devem ser considerados na gestão de medicamentos em hospitais de alta complexidade.

Abstract
Introduction: The therapy using molecular-targeted (MTT) and monoclonal antibodies (MA) are examples of new therapeutic technologies, in search of greater clinical effectiveness and reduction of adverse effects in the fight against highly frequent diseases. Generally, new technologies impact in high costs. OBJECTIVES: evaluate the financial impact generated by the use of molecular-targeted and monoclonal antibodies therapies in the teaching Hospital de Clínicas, Porto Alegre, Brazil. Methods: The first 60 higher monetary spending drug items of the last 12 months were analyzed. From them, drugs which fit in the categories under study and have been regularly used were identified. The monthly expenditures with each item were tabulated and compared with the total expenditures on drugs, in order to calculate the budgetary impact. The major groups of diseases treated with each agent were analyzed. Results: Two MTTs agents (gefitinib and infliximab) and three ACMs (rituximab, basiliximab and abciximab) were identified. The highest expenditure items, respectively, per year, were the oncological medicine gefitinib (US$ 72.228.80), followed by immunosuppressives basiliximab (US$ 97.138.50) and infliximab (US$ 77.534.90), the oncological rituximab (US$ 72.228.80) and the platelet aggregation inhibitor abciximab (US$ 54.090.10) corresponding to, respectively, 10.4%, 9.3%, 7.5%, 7.0% and 5.3% of total drug expenditure per year (US$ 13.403.300). Trastuzumab, bortezomib and imatinib were often used, but directly supplied by the public system, in a way that didn´t impacted directly in the hospital budgetary management. Conclusions: The MTT and MA have important budget impact and are mainly used to treat some types of cancer, cardiovascular disease and immunosuppressive therapies. These aspects should be considered in the management of drugs in hospitals of high complexity.
