Introdução: Um hospital de referência, especializado em cardiologia, padronizou clopidogrel, ticagrelor e recentemente aprovou a incorporação do prasugrel com uma estimativa de quinze pacientes com implante de stent mensalmente. A trombose de stent é uma complicação rara, mas grave, e recomenda-se que os pacientes sejam tratados com terapia antiplaquetária. Considerando as opções terapêuticas existentes e a baixa adesão ao tratamento, foi solicitado um estudo de custos para discutir uma proposta de desinvestimento. Métodos: Perspectiva do hospital; População: pacientes com infarto do miocárdio e implante de stent; Intervenção: prasugrel; Comparadores: clopidogrel e ticagrelor; Desfechos de interesse: trombose de stent e sangramento major; e tipos de estudo: meta-análises e ensaios clínicos randomizados. Revisão da literatura realizada na base de dados Medline, via Pubmed e realizada meta-análise. Estimativas de custos: coleta de dados realizada no banco de dados da instituição. Resultados: Treze artigos foram selecionados; não há diferenças nos desfechos para prasugrel e ticagrelor; a descontinuação do tratamento é o fator de risco mais importante para trombose de stent e o sangramento major é um importante preditor de não adesão ao tratamento. Análise quantitativa: três ensaios clínicos. Todos mostraram superioridade de prasugrel e ticagrelor sobre o clopidogrel para o desfecho de trombose de stent (Odds Ratio, OR 0,60 Intervalo de Confiança, IC 95 por cento [.40; .90]), o que foi comprometido pela heterogeneidade dos estudos (I2 = 64 por cento); e desfecho favorável para clopidogrel em comparação com ticagrelor e prasugrel para o desfecho de sangramento major (OR 1,28 IC 95 por cento [1,10; 1,50]). Custo anual com o tratamento dos quinze pacientes: clopidogrel = USD5.765,86; prasugrel = USD41.047,48; e ticagrelor = USD44.081,30. Conclusões: As evidências encontradas sustentam a abertura de um processo de desinvestimento e sugerem aos gestores da instituição uma reflexão sobre: estratégia para otimizar a adesão ao tratamento; e especialmente no custo de oportunidade com novas tecnologias cerca de 10 vezes mais caras.

Abstract
Introduction: A reference hospital, specialized in cardiology, has standardized clopidogrel, ticagrelor and recently the incorporation of prasugrel was approved with an estimate of fifteen patients with stent implantation monthly. Stent thrombosis is a rare but serious complication and it is recommended that patients be treated with antiplatelet therapy. Considering the existing therapeutic options and the low adherence to treatment, a cost study was requested to discuss a disinvestment proposal. Methods: Perspective of the hospital; Population: patients with myocardial infarction and stent implantation; Intervention: prasugrel; Comparators: clopidogrel and ticagrelor; Outcomes of interest: stent thrombosis and major bleeding; and types of study: meta-analyzes and randomized clinical trials. Literature review was performed in the Medline database, via Pubmed and performed metaanalysis. Cost estimates: data collection made in the institution’s database. Results: Thirteen articles were selected; there are no differences in the outcomes for prasugrel and ticagrelor; discontinuation of treatment is the most important risk factor for stent thrombosis and major bleeding is an important predictor of nonadherence to treatment. Quantitative analysis: three clinical trials. All showed a superiority of prasugrel and ticagrelor over clopidogrel for outcome of stent thrombosis (Odds Ratio, OR 0.60 Confidence Interval, CI 95 percent [.40; .90]), which was committed by the heterogeneity of studies (I2 = 64 percent); and favorable outcome for clopidogrel compared to ticagrelor and prasugrel for the outcome of major bleeding (OR 1.28 CI 95 percent [1.10; 1.50]). Annual cost with treatment of the fifteen patients: clopidogrel = USD5,765.86; prasugrel = USD41,047.48; and ticagrelor = USD44,081.30. Conclusions: The evidences found support the opening of a disinvestment process and suggest to the managers of the institution a reflection on: strategy to optimize adherence to treatment; and especially in the cost of opportunity with new technologies about 10 times more expensive
