Introdução: A organização do Sistema Único de Saúde (SUS) brasileiro emerge de uma arquitetura legal que define a estrutura e o financiamento de tecnologias e serviços de saúde, bem como as responsabilidades dos gestores nas três esferas de governo (federal, estadual e municipal). Nesse contexto, a avaliação e incorporação de tecnologias, bem como a elaboração e publicação de Diretrizes de Práticas Clínicas (DPC), são centralizadas no nível federal enquanto sua implementação é executada no nível local. Assim, há o grande desafio de integrar e coordenar as ações para implementação das DPC. Objetivo: Avaliar as instruções desenvolvidas no nível federal que abordam a implementação das DPC no SUS. Métodos: Foi realizada análise documental de instruções publicadas entre 2009 e 2021, que fornecem diretrizes para a implementação das DPC no Brasil. Resultados: Foram identificados 18 documentos, dos quais apenas sete apresentaram referências diretas ou indiretas à implementação. De modo geral, as diretrizes sobre implementação das DPC no SUS são incipientes. Discussão: Embora os gestores locais tenham autonomia para conduzir a implementação de tecnologias e guias de prática clínica, é possível que eles não tenham recursos técnicos capazes de estruturar estratégias de implementação, o que pode causar falta de padronização e fragilizar a cadeia de gestão, além de perda de recursos investidos no processo. Assim, oportunidades de diálogo e orientação no nível federal têm o potencial de auxiliar e facilitar a implementação de tecnologias em saúde e práticas clínicas, reduzindo a lacuna entre o planejamento e o uso efetivo das CPGs no SUS.

Abstract
Background: The Brazilian Public Health System (SUS) organization emerges from a legal architecture that defines the structure and the financing of health technologies and services, as well as the responsibilities of managers in three spheres of government (federal, state and municipal). In this context, the evaluation and incorporation of technologies, as well as the development and publication of Clinical Practice Guidelines (CPG), are centralized at the federal level while their implementation is executed at the local level. Thus, there is the great challenge of integrating and coordinating actions for implementation of CPG. Objective: To evaluate instructions developed at the federal level that address the implementation of CPG in SUS. Methods: Documentary analysis was performed of instructions published between 2009 and 2021, that provide guidelines for the implementation of CPG in Brazil. Results: Eighteen documents were identified, of which only seven presented direct or indirect references to implementation. In general, guidelines on CPG implementation in SUS are incipient. Discussion: Although local managers have autonomy to conduct the implementation of technologies and clinical practice guides, it is possible that they don’t have technical resources capable of structuring implementation strategies, which can cause lack of standard and weaken the management chain, in addition to loss of resources invested in the process. Thus, opportunities for dialogue and guidance at the federal level have the potential to assist and to facilitate the implementation of health technologies and clinical practices, reducing the gap between planning and the effective use of CPGs in SUS.
