Contexto e Introdução: Em 2017, a Comissão Nacional de Incorporação de Tecnologia – Conitec aprovou diretrizes para tratamentos de mucopolissacaridose tipo 1 (MPS1) e mucopolissacaridose tipo 2 (MPS2), permitindo acesso a tecnologias custosas que eram obtidas apenas por via judicial neste país. Objetivos/Meta: Estimar o custo incremental da inclusão de Laronidase e Idursulfase para tratamentos de MPS1 e MPS2 no Sistema Único de Saúde (SUS), respectivamente. Métodos: Utilizando dados de aquisições de tratamentos pelo Ministério da Saúde para liquidação de processos médicos, incidência e prevalência e peso médio dos pacientes da literatura e o custo de aquisição de medicamentos por via judicial em 2017. Foi construída uma comparação entre o custo de fornecimento de tratamento por via judicial em 2017 e uma projeção do custo total após a incorporação dos tratamentos ao SUS em 2018. Resultados e Discussão: O custo incremental estimado da inclusão desses tratamentos no SUS será de R$ 108.916.163,45 (R$ 20.463.307,77 com Laronidase e R$ 88.452.854,68 com Idursulfase), o que representa 1,49% do orçamento total para aquisição de medicamentos de alto custo do Ministério da Saúde, considerando que 41,8% dos pacientes estimados iniciarão o tratamento após a implementação da diretriz, com economia de custos estimada em cerca de R$ 18 milhões. Implicações para desenvolvedores/usuários de diretrizes: Estimar o custo de incorporação das tecnologias nas diretrizes ajuda os formuladores de políticas a garantir a implementação nos sistemas universais por meio da redução do custo e da expansão do acesso à saúde. Conclusão As diretrizes para o MPS1 e MPS2 permitiram um padrão de assistência à saúde, expandindo o acesso ao tratamento e economizarão mais de R$ 18 milhões para o SUS em 2018.

Abstract
Background & Introduction: In 2017, the National Committee for Technology Incorporation – Conitec approved guidelines for mucopolysaccharidosis type 1 (MPS1) and mucopolysaccharidosis type 2 (MPS2) treatments, allowing access to costly technologies that were obtained only by judicial mean in this country. Objectives/Goal: Estimate the incremental cost of the inclusion of Laronidase and Idursulfase for MPS1 and MPS2 treatments in the Brazilian Public Health System (SUS), respectively. Methods: Using data from acquisitions of treatments by the Ministry of Health to settle medical lawsuits, incidence and prevalence and average weight of the patients from the literature and the cost of acquiring medicines by judicial means in 2017. A comparison was built between the cost of providing treatment by judicial means in 2017, and a projection of the overall cost after treatments were incorporated the SUS in 2018. Results & Discussion: The estimated incremental cost of including these treatments in SUS will be of R$108.916.163,45 (R$20.463.307,77 with Laronidase and R$88.452.854,68 with Idursulfase) which represents 1,49% of the total budget to acquire high-cost medicines of the Ministry of Health, considering the 41,8% of estimated patients will start treatment after guideline implementation, with estimated cost savings of about R$18 million. Implications for guideline developers/users: Estimate the cost of incorporation the technologies in the guidelines helps policy-makers to guarantee the implementation in the universal systems through of reducing the cost and expanding the access to health. Conclusion The guidelines for the MPS1 and MPS2 allowed a standard of the health assistance expanding the access to treatment and will save more than R$18 million to SUS in 2018.
