Contexto e introdução: A adaptação das diretrizes fornece uma solução alternativa em vista da redução de recursos financeiros e humanos e restrições de tempo, tornando o processo mais eficiente e evitando a duplicação de esforços. No entanto, é necessário considerar as diferenças culturais e organizacionais no novo cenário para garantir a aplicabilidade na prática. Objetivos/meta: Comparar as recomendações do American College of Rheumatology (ACR) para o tratamento da artrite reumatoide (RA) inicial e as recomendações adaptadas para a Região do Mediterrâneo Oriental (EMR) e o Brasil. Métodos: Usamos a abordagem GRADE-Adolopment para adaptar a diretriz de tratamento de AR do ACR de 2015 para o EMR e o Brasil. A diretriz de origem abordou 15 questões. Para as diretrizes do EMR e do Brasil, 8 questões foram priorizadas, 6 das quais eram as mesmas para ambas as diretrizes. Resultados e discussão: As recomendações para as 6 questões cobertas pelo EMR e pelas diretrizes brasileiras concordaram tanto em direção quanto em força. Todas as recomendações estavam na mesma direção que as recomendações da diretriz do ACR, mas a força de 3 recomendações mudou de forte para condicional na diretriz brasileira e de 5 mudou de forte para condicional na diretriz do EMR. Recomendações condicionais foram feitas com base em questões de custo, viabilidade e impacto nas iniquidades em saúde. Embora todas as recomendações estivessem na mesma direção, houve 50% de discordância entre as diretrizes originais e adaptadas em relação à força das recomendações. Implicações para desenvolvedores/usuários de diretrizes: A adoção de recomendações pode não ser apropriada ao lidar com diferentes cenários. Um método de adaptação como o GRADE-Adolopment é preferível porque permite adaptar as recomendações a questões locais, como custos, valores e preferências e equidade.

Abstract
Background & Introduction: Guideline adaptation provides an alternative solution in view of reduced financial and human resources and time constraints, making the process more efficient and avoiding duplication of efforts. However, it is necessary to consider the cultural and organizational differences in the new setting to ensure applicability in practice. Objectives/Goal: To compare the American College of Rheumatology (ACR) recommendations for the treatment of early rheumatoid arthritis (AR) and adapted recommendations for the Eastern Mediterranean Region (EMR) and Brazil. Methods: We used the GRADE-Adolopment approach to adapt the 2015 ACR RA treatment guideline to the EMR and Brazil. The source guideline addressed 15 questions. For the EMR and Brazil guidelines, 8 questions were prioritized, 6 of which were the same for both guidelines. Results & Discussion: The recommendations for the 6 questions covered by the EMR and Brazilian guidelines agreed both in direction and strength. All recommendations were in the same direction as the ACR guideline recommendations, but the strength of 3 recommendations changed from strong to conditional in the Brazilian guideline and of 5 changed from strong to conditional in the EMR guideline. Conditional recommendations were made based on cost issues, feasibility, and impact on health inequities. Although all recommendations were in the same direction, there was 50% disagreement between the original and adapted guidelines regarding the strength of recommendations. Implications for guideline developers/users: Adoption of recommendations may not be appropriate when dealing with different settings. An adaptation method such as GRADE-Adolopment is preferred because it allows tailoring the recommendations to local issues, such as costs, values and preferences, and equity.
