Contexto: Diretrizes de Prática Clínica (CPGs) baseadas em evidências incluem recomendações que visam otimizar o atendimento ao paciente. A aplicação de uma CPG é um processo complexo que depende de múltiplos fatores, considerando as características do cenário de implementação e as barreiras e facilitadores da mudança. Atualmente, não há revisões sobre o tópico com foco na Região da América Latina. Objetivos: Identificar as barreiras e facilitadores para a implementação de Diretrizes de Prática Clínica na América Latina. Relevância e importância para os pacientes: Identificar barreiras e facilitadores pode auxiliar na implementação de cuidados de saúde centrados no paciente e informados por evidências na Região. Métodos: Nossa revisão de escopo segue a orientação do JBI, e o protocolo foi registrado (osf.io/rs23x). Seguimos a abordagem População, Contexto e Conceito (PCC) para definir a elegibilidade: profissionais de saúde da América Latina (todos os níveis de ambientes de saúde) e barreiras e facilitadores. Estudos primários qualitativos, quantitativos ou de métodos mistos foram incluídos. Revisões narrativas, estudos de ferramentas de implementação, estudos que descrevem a prática atual de CPG na região da América Latina, a extensão da conformidade com CPGs ou a atitude do clínico em relação às diretrizes sem descrever as barreiras ou facilitadores foram excluídos. Fizemos buscas no MEDLINE e LILACS em fevereiro de 2024. Resultados: Esta é uma revisão em andamento. Incluímos preliminarmente quatro estudos. Três usaram uma abordagem qualitativa, um da Colômbia e dois do México. Um estudo quantitativo foi encontrado, que relata barreiras à implementação de um CPG no Brasil, Venezuela e México, com foco nas diferenças em ambientes clínicos em comparação com o desenvolvimento do CPG. Atualmente, estamos mapeando as principais barreiras e facilitadores identificados nos estudos incluídos, que até agora incluem: estigma, falta de integração, ignorância do CPG, resistência à implementação, recursos limitados e falta de treinamento. Ter o CPG impresso no ponto de atendimento e recomendações fáceis de seguir foram identificados como facilitadores. Conclusões: Os resultados completos da revisão serão apresentados no GES 2024. Forneceremos uma visão geral das lacunas atuais de conhecimento e evidências sobre este tópico, especialmente as necessidades de pesquisa para promover ainda mais a implementação do uso de CPG na região.

Abstract
Background: Evidence-based Clinical Practice Guidelines (CPGs) include recommendations that aim to optimise patient care. Applying a CPG is a complex process that depends on multiple factors, considering the characteristics of the implementation setting and the barriers and enablers of change. There are currently no reviews on the topic focusing on the Latin American Region. Objectives: Identify the barriers and enablers for implementing Clinical Practice Guidelines in Latin America. Relevance and importance to patients: Identifying barriers and enablers can aid the implementation of patient-centred and evidence-informed healthcare in the Region. Methods: Our scoping review follows the guidance of the JBI, and the protocol was registered (osf.io/rs23x). We followed the Population, Context, and Concept (PCC) approach for defining eligibility: healthcare professionals from the Latin America (all levels of healthcare settings) and barriers and enablers. Qualitative, quantitative or mixed methods primary studies were included. Narrative reviews, studies of implementation tools, studies that describe the current CPG practice in the Latin America region, the extent of compliance with CPGs, or clinician attitude toward the guidelines without describing the barriers or enablers were excluded. We searched MEDLINE and LILACS in February 2024. Results: This is an ongoing review. We have preliminarily included four studies. Three used a qualitative approach, one from Colombia and two from Mexico. One quantitative study was found, which reports barriers to implementing one CPG in Brazil, Venezuela, and Mexico, focusing on the differences in clinical settings compared with the CPG development. We are currently mapping the main barriers and enablers identified in the included studies, which so far include: stigma, lack of integration, ignorance of CPG, resistance to the implementation, limited resources, and lack of training. Having the CPG printed at the point of care and easy-to-follow recommendations were identified as enablers. Conclusions: The full results of the review will be presented at the GES 2024. We will provide an overview of the current knowledge and evidence gaps on this topic, especially the research needs to promote further the implementation of the use of CPG in the region.
