Resumo
Introdução (com objetivo): A Assistência Farmacêutica em Oncologia (AF-ONCO) no SUS passa por uma transição que inclui um modelo de financiamento centralizado. Este modelo visa a efetivação da oferta de medicamentos incorporados pela Comissão Nacional de Incorporação de Tecnologias no SUS (CONITEC) mediante a criação de procedimentos específicos na Tabela SIGTAP, financiados inicialmente pelo Fundo de Ações Estratégicas e Compensação (FAEC). Estimar o impacto orçamentário anual da compra centralizada de dez medicamentos oncológicos estratégicos e de alto custo e analisar a projeção de gastos do Ministério da Saúde para o exercício de 2026 sob o novo modelo de financiamento. Material e Método: Estudo descritivo de impacto orçamentário baseado em dados técnicos de incorporação de procedimentos na Tabela SIGTAP/AF-ONCO. Foram analisados os valores de impacto anual total para atendimento da demanda total ou progressiva de dez fármacos (Acetato de lanreotida, Brigatinibe, Carfilzomibe, Lenalidomida, Nivolumabe, Olaparibe, Sunitinibe, Rituximabe e Trastuzumabe) estimados para o Plano Orçamentário 2026. Discussão: A análise dos medicamentos selecionados indica um impacto para atendimento da demanda total de R$ 1.205.562.324,83 (R$ 1.2 BI) e um de R$ 742,2 milhões para atendimento progressivo. O Nivolumabe apresenta o maior impacto financeiro individual (R$ 409,9 milhões), seguido pelo Carfilzomibe (R$ 268,8 milhões) e pelo Olaparibe (R$ 180,1 milhões), evidenciando a alta concentração de recursos em terapias de alvo molecular e imunoterápicos. Estima-se que a incorporação dessas tecnologias beneficiará diretamente cerca de X pacientes. Conclusões: Os dados revelam uma elevada concentração de recursos em imunoterápicos e terapias de alvo molecular, com o Nivolumabe isoladamente representando 33,9% do impacto anual total projetado. A discrepância entre o impacto total (R$ 1,2 bi) e o market-share inicial (R$ 742 mi) ressalta o desafio da transição epidemiológica e a velocidade de adoção dessas tecnologias no SUS. Medicamentos como o Carfilzomibe (R$ 268,8 mi) e Olaparibe (R$ 180 mi) demonstram o alto custo unitário do tratamento de neoplasias hematológicas e ginecológicas, respectivamente. A centralização do financiamento busca mitigar as disparidades regionais na oferta, mas a sustentabilidade do modelo depende da precisão das estimativas de market-share, visto que subestimar a demanda pode gerar desabastecimento ou pressão orçamentária imprevista no fundo FAEC antes da consolidação do teto MAC. Conclusão: A centralização do financiamento via AF-ONCO permite uma regulação mais estrita da oferta de antineoplásicos. Contudo, o expressivo impacto orçamentário projetado para 2026 (R$ 1.2 BI) exige um planejamento fiscal robusto, visto que a incorporação de novas tecnologias oncológicas demanda uma alocação de recursos EXPRESSIVA.

Abstract
Introduction (with objective): Pharmaceutical Assistance in Oncology (AF-ONCO) within the Brazilian Unified Health System (SUS) is undergoing a transition that includes a centralized financing model. This model aims to ensure the effective provision of medications incorporated by the National Commission for the Incorporation of Technologies in the SUS (CONITEC) through the creation of specific procedures in the SIGTAP Table, initially financed by the Strategic Actions and Compensation Fund (FAEC). The objective is to estimate the annual budgetary impact of the centralized purchase of ten strategic and high-cost oncology medications and to analyze the Ministry of Health's projected expenditures for the fiscal year 2026 under the new financing model. Material and Method: Descriptive study of budgetary impact based on technical data on the incorporation of procedures in the SIGTAP/AF-ONCO Table. The total annual impact values for meeting the total or progressive demand for ten drugs (Lanreotide acetate, Brigatinib, Carfilzomib, Lenalidomide, Nivolumab, Olaparib, Sunitinib, Rituximab, and Trastuzumab) estimated for the 2026 Budget Plan were analyzed. Discussion: The analysis of the selected drugs indicates an impact of R$ 1,205,562,324.83 (R$ 1.2 billion) for meeting the total demand and R$ 742.2 million for progressive demand. Nivolumab presents the highest individual financial impact (R$ 409.9 million), followed by Carfilzomib (R$ 268.8 million) and Olaparib (R$ 180.1 million), highlighting the high concentration of resources in molecularly targeted therapies and immunotherapies. It is estimated that the incorporation of these technologies will directly benefit approximately X patients. Conclusions: The data reveal a high concentration of resources in immunotherapies and molecularly targeted therapies, with Nivolumab alone representing 33.9% of the total projected annual impact. The discrepancy between the total impact (R$ 1.2 billion) and the initial market share (R$ 742 million) highlights the challenge of the epidemiological transition and the speed of adoption of these technologies in the SUS (Brazilian Unified Health System). Medications such as Carfilzomib (R$ 268.8 million) and Olaparib (R$ 180 million) demonstrate the high unit cost of treatment for hematological and gynecological neoplasms, respectively. Centralizing funding seeks to mitigate regional disparities in supply, but the sustainability of the model depends on the accuracy of market share estimates, since underestimating demand can lead to shortages or unforeseen budgetary pressure on the FAEC fund before the consolidation of the MAC ceiling. Conclusion: Centralizing funding through AF-ONCO allows for stricter regulation of the supply of antineoplastic drugs. However, the significant projected budgetary impact for 2026 (R$ 1.2 billion) requires robust fiscal planning, since the incorporation of new oncological technologies demands a substantial allocation of resources.
