Resumo
Objetivos: Avaliar o padrão de profilaxia para tromboembolismo venoso (TEV) em artroplastia total do joelho (ATJ) e artroplastia total do quadrilheiro (ATQ) no contexto do sistema de saúde Brasileiro. Métodos: Estudo retrospectivo de revisão de prontuários médicos de pacientes submetidos a ATJ ou ATQ eletivas em 2010. Todos os prontuários eram de um hospital público ou dois hospitais privados em São Paulo, Brasil. Pacientes com 18 anos ou mais foram incluídos, excluindo-se aqueles que utilizaram medicamentos antitrombóticos antes da cirurgia. A análise descritiva foi realizada por meio de distribuições de frequência e média/desvio padrão (DP). Resultados: De um total de 233 pacientes, 215 (92,3%) foram incluídos no estudo: 121 (56,3%) ATJ e 94 (43,7%) ATQ. A idade média foi de 68,2 anos (DP 9,8) e 75,2% eram do sexo feminino no grupo de ATJ, enquanto no grupo de ATQ, a idade média foi de 56,3 anos (DP 15,6) e 53,2% eram do sexo feminino. No hospital público, 71/81 (87,7%) pacientes receberam profilaxia medicamentosa, sendo a enoxaparina a escolha de tratamento. O tempo médio de início da enoxaparina foi de 29 horas após ambos os tipos de cirurgia. A duração média do tratamento hospitalar foi de 3 dias (ATJ) e 4 dias (ATQ). Nos hospitais privados, 132/134 (98,5%) receberam profilaxia. O tratamento de escolha foi enoxaparina em 130 (98,5%), enquanto dabigatrano e meias de compressão foram usadas em um caso cada. As meias de compressão representaram profilaxia adjuvante em 16,7%. O tempo médio de início foi de 20 horas após ATJ e 17 horas após ATQ para enoxaparina, enquanto 46 horas para dabigatrano. A duração média do tratamento hospitalar foi de 4 dias (enoxaparina) e 3 dias (dabigatrano). Conclusões: As taxas de prevenção intra-hospitalar de TEV neste estudo estão dentro da prática internacional (ENDORSE 2008), com taxas mais baixas no público. Existem incertezas sobre se os pacientes ambulatoriais receberam o mesmo nível de profilaxia para TEV. É importante continuar a conscientização sobre o impacto clínico e econômico do TEV após cirurgias ortopédicas, principalmente no atendimento ambulatorial.

Patients were 18+ years old, and excluded from the study if antithrombotic drugs were used prior to surgery. Descriptive analysis was performed through frequency distributions and mean/standard deviation (SD). Results: From a total of 233 patients, 215 (92.3%) were included in the study: 121 (56.3%) TKA and 94 (43.7%) THA. Mean age was 68.2 (SD 9.8) years and 75.2% were female in the TKA group while, in the THA group mean age was 56.3 yrs (SD 15.6) and 53.2% were female.. From the public hospital, 71/81 (87.7%) patients received drug prophylaxis, being enoxaparin the choice of treatment. Enoxaparin onset mean time was 29hs after both surgery types. Mean inpatient treatment duration was 3 (TKA) and 4 days (THA). In private hospitals 132/134 (98.5%) received prophylaxis. Treatment of choice was enoxaparin in 130 (98.5%), while dabigatran and compression stocking in one case each. Compression stocking represented adjuvant prophylaxis in 16.7%. Onset mean time was 20hs after TKA and 17hs after THA for enoxaparin while 46hs for dabigatran. Mean inpatient treatment duration was 4 days(enoxaparin) and 3 days (dabigatran). Conclusions: The intra-hospitalar prevention rates of VTE in this study is within international (ENDORSE 2008), practice, while with lower rates in the public. There are uncertainties that outpatients received the same level of VTE prophylaxis. It is important to continue the awareness of the clinical and economic impact of VTE after orthopedic surgery, mainly in the outpatient care.
