Objetivos: No sistema de saúde privado Brasileiro, a cobertura para terapia oral não é obrigatória. A associação de lapatinibe e capecitabina (LAP/CAP), ambos administrados oralmente, é uma combinação eficaz para o tratamento de pacientes com câncer de mama metastático HER2 positivo (MBC-HER2+) após progressão para trastuzumabe, mas não é frequentemente utilizada. O objetivo deste estudo foi avaliar os padrões de tratamento e os custos associados ao MBC-HER2+ após progressão para trastuzumabe, sob a perspectiva do sistema de saúde privado Brasileiro, e calcular o impacto econômico associado ao uso de LAP/CAP para essa população. Métodos: A base de dados Evidencias® contém informações sobre cerca de 3.000.000 vidas cobertas por planos de saúde privados no Brasil. Nesta base de dados, foram selecionados pacientes diagnosticados com MBC-HER2+ e tratados com quimioterapia após falha ao trastuzumabe. Para cada paciente, os custos do tratamento foram calculados considerando apenas os custos dos medicamentos. Três cenários de custo foram estimados: i) quimioterapia (CT) real utilizada pelos pacientes; ii) uso hipotético de LAP/CAP; iii) uso hipotético de trastuzumabe associado à capecitabina (TRAST/CAP). Resultados: Um total de 182 pacientes foi incluído na análise. A duração média do tratamento foi de 10,5 semanas (IC 95%: 8,9 - 12,2). Vinte e dois protocolos de tratamento foram identificados após falha ao trastuzumabe, principalmente: paclitaxel (27%), docetaxel (20%) e vinorelbine (14%). O custo médio por paciente foi de R$19.114,68, R$25.977,61 e R$39.437,26, nos cenários de CT, LAP/CAP projetado e TRAST/CAP, respectivamente. Considerando os resultados segmentados por protocolo de CT, os custos com LAP/CAP foram inferiores aos da CT para pacientes tratados com paclitaxel associado ou não a gemcitabina, entre outros. Para todos os pacientes, os custos projetados com LAP/CAP foram menores do que com TRAST/CAP. Conclusões: Pacientes tratados com CT, elegíveis para tratamento com LAP/CAP ou TRAST/CAP, apresentaram menores custos de tratamento quando comparados a LAP/CAP ou TRAST/CAP. Em algumas combinações específicas de CT, no entanto, LAP/CAP pode resultar em menores custos, sendo considerada uma alternativa economicamente viável.

Objetives: In the Brazilian private healthcare system, coverage for oral therapy is not mandatory. The association of lapatinib and capecitabine (LAP/CAP), both orally administered, is an effective combination for the treatment of patients with metastatic HER2 positive breast cancer (MBC-HER2+) after progression to trastuzumab, but is not frequently used. The Objetive of this study was to evaluate treatment patterns and associated costs of MBC-HER2+ after progression to trastuzumab, under the perspective of the Brazilian private healthcare system and to calculate the economic impact associated with using LAP/CAP for this population. Methods: Evidencias® database contains data related to around 3,000,000 lives covered by private health plans in Brazil. In this database, patients diagnosed with MBC-HER2+ and treated with chemotherapy after failure to trastuzumab were selected. For every patient, treatment costs were calculated considering drug costs only. Three cost scenarios were estimated: i) real-life chemotherapy (CT) used by patients; ii) hypothetical use of LAP/CAP; iii) hypothetical use of trastuzumab associated to capecitabine (TRAST/CAP). Results: A total of 182 patients were included in the analysis. Average treatment duration was 10.5 weeks (95% CI: 8.9 - 12.2). Twenty-two treatment protocols were identified after failure to trastuzumab, mainly: paclitaxel (27%), docetaxel (20%) and vinorelbine (14%). The average cost per patient were R$19,114.68, R$25,977.61 and R$39,437.26, in the CT, and projected LAP/CAP and TRAST/CAP scenarios, respectively. Considering the Results segmented by CT protocol, LAP/CAP costs were lower than CT for patients treated with paclitaxel associated or not to gemcitabine, among others. For all patients, projected costs with LAP/CAP were lower than TRAST/CAP. Conclusions: Patients treated with CT, eligible for treatment with LAP/CAP or TRAST/CAP, had lower treatment costs when compared to LAP/CAP or TRAST/CAP. In some specific CT combinations, however, LAP/CAP may result in lower costs, thus being considered an economically viable alternative.
