Objetivos: O uso associado de antitrombóticos, antiplaquetários e estratégias invasivas em pacientes com síndrome coronariana aguda sem elevação do segmento ST (NSTE-ACS) reduz eventos cardiovasculares, porém aumenta o risco de sangramentos. Estudos clínicos mostraram que o fondaparinux é tão eficaz quanto a enoxaparina no tratamento de pacientes com NSTE-ACS, mas com menor risco de eventos hemorrágicos. O objetivo foi determinar a relação custo-efetividade do fondaparinux versus enoxaparina em pacientes com NSTE-ACS no Brasil, sob a perspectiva do Ministério da Saúde (MoH). Métodos: Foi conduzido um modelo analítico de árvore de decisão para estimar os custos e consequências das terapias direcionadas em pacientes com NSTE-ACS. Os dados de entrada do modelo derivaram do estudo OASIS-5 (N=20.078 pacientes com NSTE-ACS randomizados para fondaparinux ou enoxaparina). O desfecho analisado foi um composto de eventos cardiovasculares (ou seja, morte, infarto agudo do miocárdio, acidente vascular cerebral e sangramentos maiores). O horizonte temporal do modelo foi de 9, 30 e 180 dias após o NSTE-ACS. Os custos diretos dos eventos e tratamentos de NSTE-ACS foram calculados (ou seja, medicamentos, angiografia coronária, revascularização miocárdica, intervenção percutânea - PCI, hospitalizações, etc.). Os custos foram expressos em moeda Brasileira de 2010 (1BRL=0,59USD). Análises univariadas e multivariadas (Monte Carlo) testaram a robustez do modelo. Resultados: No dia 9, o custo médio por paciente tratado foi de 2.575 BRL para fondaparinux e 2.688 BRL para enoxaparina. Mais de 65% dos custos totais foram atribuídos ao tratamento invasivo (PCI e revascularização). Os custos com medicamentos (terapias hospitalares) representaram 10% (fondaparinux) e 12% (enoxaparina) dos custos totais. As taxas estimadas de eventos cardiovasculares foram 7,3% e 9,0% para fondaparinux e enoxaparina, respectivamente. Os resultados se mantiveram inalterados aos 30 e 180 dias pós-NSTE-ACS. Análise de sensibilidade confirmou os resultados base. Conclusões: Fondaparinux foi dominante em relação à enoxaparina (custos mais baixos, melhores benefícios a longo prazo). O impacto orçamentário após 5 anos de substituição do anticoagulante (com taxa constante de adoção de 20% ao ano) poderia alcançar 90 milhões de BRL em economias para o Ministério da Saúde e o sistema de saúde Brasileiro.

Objetives: Associated use of antithrombotics, antiplatelets and invasive strategies in patients with non ST-segment elevation acute coronary syndromes (NSTE-ACS) reduces cardiovascular events, however, with an increase in the risk of bleeding. Clinical studies showed that fondaparinux is as effective as enoxaparin in treating patients with NSTE-ACS, but with reduced risk of bleeding events. The Objetive was to determine the cost-effectiveness of fondaparinux versus enoxaparin in patients with NSTE-ACS in Brazil from the perspective of the Brazilian Ministry of Health (MoH). Methods: An analytic decision tree model was conducted to estimate the resultant costs and consequences of the targeted therapies in patients with NSTE-ACS. Model input data derived from the OASIS-5 study (N=20,078 NSTE-ACS patients randomized to fondaparinux or enoxaparin). The analyzed outcome was a composite of cardiovascular events (i.e., death, acute myocardial infarction, stroke, and major bleedings). Model time horizon was 9, 30, and 180 days post-NSTE-ACS. Direct costs of NSTE-ACS events and treatments were computed (i.e., drugs, coronary angiography, myocardial revascularization, percutaneous intervention - PCI, hospitalizations, etc.). Costs were expressed in 2010 Brazilian currency (1BRL=0.59USD). Univariate and multivariate (Monte Carlo) analyses tested model robustness. Results: At day 9, the average cost per patient treated was 2,575 for fondaparinux and 2,688 for enoxaparin. Over 65% of total costs were attributed to the invasive treatment (PCI and revascularization). Drug costs (in-hospital therapies) accounted for 10% (fondaparinux) and 12% (enoxaparin) of total costs. The estimated rates of cardiovascular events were 7.3% and 9.0% for fondaparinux and enoxaparin, respectively. Results kept unchanged on days 30 and 180 post-NSTE-ACS. Sensitivity analysis confirmed base-case Results. Conclusions: Fondaparinux was dominant over enoxaparin (lower costs, better long-term benefits). The budget impact after 5 years of anticoagulant substitution (at 20% constant adoption rate per year) could reach 90 million BRL in savings for the Brazilian MoH and healthcare system.
