Objetivos: Estimar o custo do tratamento da dor oncológica com quatro opioides fortes (metadona, morfina, oxicodona, fentanil) no sistema de saúde privado Brasileiro entre janeiro e dezembro de 2010. Métodos: Foi analisado um banco de dados de reivindicações de mais de 57 HMOs para recuperar o uso combinado de opioides fortes e tratamentos oncológicos entre janeiro e dezembro de 2010. Os registros mostraram gastos com medicamentos, materiais, hospitalização, procedimentos e diagnósticos. Resultados: Durante o período de um ano, 293.918 pacientes utilizaram pelo menos um dos quatro opioides. O gasto total com esses pacientes foi de R$ 3.243.890.302,91 (R$ 11.036,72/paciente/ano). Cerca de 53% desses pacientes (157.104) fizeram uso concomitante de tratamentos oncológicos, representando cerca de 74% dos custos totais (R$ 2.424.503.643,76), com um custo médio de R$ 15.432,48/paciente/ano. Os pacientes restantes (136.814) tiveram um custo médio de R$ 5.989,06 por paciente/ano. Dentro da população de pacientes oncológicos, o gasto total com os quatro opioides foi de R$ 5.203.001,81. O fentanil foi o opioide mais utilizado em cerca de 66% dos pacientes, seguido pela morfina (33%), metadona (1%) e oxicodona (0,8%). Cerca de 17% da população de pacientes oncológicos fez uso de dois ou mais opioides durante o período de estudo. Conclusões: O tratamento da dor em pacientes oncológicos é mais oneroso para os pagadores privados no Brasil em comparação com pacientes que não recebem tratamento oncológico. Embora 47% dos pacientes fossem considerados não oncológicos, isso não é certo, pois eles poderiam ter recebido tratamento oncológico fora do período do estudo ou em um prestador não coberto pelo banco de dados (por exemplo, hospital público). Com cerca de 17% dos pacientes oncológicos recebendo dois ou mais tratamentos com opioides ao longo dos 12 meses, sugere-se que a rotação de opioides é comum.

The remaining patients (136,814) had an average cost of R$ 5.989,06 per patient/year. Within the oncology patient population, the total healthcare expenditure with the four opioids alone was R$ 5,203,001.81. Fentanyl was the most commonly used opioid in about 66% of patients, followed by morphine (33%), methadone (1%) and oxicodone (0,8%). Around 17% of the oncology patient population made use of two or more opioids during the study period. Conclusions: Pain treatment of oncology patients is more costly for private payers in Brazil when compared with patients not receiving oncological treatment. Although 47% of patients were considered non-oncological, this is not certain as they could have received oncological treatment outside the study period or in a provider not covered by the database (e.g. public hospital). With about 17% of oncological patients receiving two or more opioid treatments with the 12 month period suggests opioid rotation is common.
