Introdução: O Brasil não possui diretrizes clínicas para leucemia linfocítica crônica (LLC), sendo necessária a incorporação formal dos medicamentos ao sistema público de saúde brasileiro (SUS). Em 2023, o rituximabe associado à fludarabina e ciclofosfamida (FCR) foi avaliado e recomendado pela instituição brasileira de ATS, a Comissão Nacional de Incorporação de Tecnologias em Saúde (Conitec), para tratamento de primeira linha da LLC. Portanto, outras opções terapêuticas, como ibrutinibe + rituximabe, devem ser avaliadas, para compor uma futura diretriz clínica para LLC. Objetivos: avaliar a eficácia e a segurança do ibrutinibe associado ao rituximabe (IR) no tratamento de primeira linha da LLC em comparação ao FCR. Métodos: está sendo realizada uma revisão sistemática (RS) com buscas no MEDLINE/PubMed, EMBASE, Cochrane Library e clinicaltrial.gov. Os desfechos de interesse são sobrevida global (SG), sobrevida livre de progressão (SLP), qualidade de vida e eventos adversos graves. O risco de viés foi avaliado pela ferramenta RoB 2 e a certeza da evidência será avaliada com GRADE. Uma meta-análise do modelo de efeito aleatório será realizada se possível. O protocolo SR está registrado no banco de dados PROSPERO (CRD42023494868). Resultados: Das 1.678 referências inicialmente identificadas, três publicações relacionadas a dois RCTs foram incluídas. Os achados iniciais sugerem que, em termos de PFS, o regime IR foi significativamente mais eficaz em comparação ao regime FCR em ambos os estudos, HRs 0,37 (IC de 95%: 0,27 a 0,51) e 0,44 (IC de 95%: 0,32 a 0,60). Em relação à sobrevida global, o regime IR superou o regime FCR em um RCT (HR: 0,47 [IC 95%: 0,25 a 0,89]), mas não em outro RCT (HR: 1,01 [IC 95%: 0,61 a 1,68]). O risco global de viés foi avaliado como alguma preocupação devido à falta de alocação oculta em ambos os RCTs. Conclusões: Ambos os RCTs indicam ganhos em PFS quando IR foi usado, com um dos RCTs sugerindo benefícios significativos em sobrevida global. Essas descobertas vantajosas de IR em comparação com FCR justificam a realização de uma avaliação econômica para determinar a relação custo-efetividade do medicamento e a avaliação subsequente do Conitec. Portanto, se sua incorporação ao SUS for viável, IR poderia fazer parte da diretriz nacional de LLC.

Abstract
Background: Brazil does not have clinical guidelines for chronic lymphocytic leukemia (CLL), making it necessary for the medications to be formally incorporated into the Brazilian public health system (SUS). In 2023, rituximab associated with fludarabine and cyclophosphamide (FCR) was evaluated and recommended by the Brazilian HTA institution, the National Committee for Health Technology Incorporation (Conitec), for first-line treatment of CLL. Therefore, other therapeutic options, such as ibrutinib + rituximab, must be evaluated, to compose a future clinical guideline for CLL. Objectives: evaluate the efficacy and safety of ibrutinib associated with rituximab (IR) in the first-line treatment of CLL compared to FCR. Methods: a systematic review (SR) is being performed with searches in MEDLINE/PubMed, EMBASE, Cochrane Library and clinicaltrial.gov. Outcomes of interest are overall survival (OS), progression-free survival (PFS), quality of life and serious adverse events. The risk of bias was evaluated by RoB 2 tool and certainty of evidence will be evaluated with GRADE. A random effect model meta-analysis will be performed if it is possible. The SR protocol is registered in the PROSPERO database (CRD42023494868). Results: Of the 1,678 references initially identified, three publications related to two RCTs were included. The initial findings suggest that, in terms of PFS, the IR regimen was significantly more effective compared to the FCR regimen across both studies, HRs 0.37 (95% CI: 0.27 to 0.51) and 0.44 (95% CI: 0.32 to 0.60). Regarding OS, the IR regimen outperformed the FCR regimen in one RCT (HR: 0.47 [95% CI: 0.25 to 0.89]), but not in another RCT (HR: 1.01 [95% CI: 0.61 to 1.68]). The global risk of bias was evaluated as some concern because of the lack of concealed allocation in both RCTs. Conclusions: Both RCTs indicate gains in PFS when IR was used, with one of the RCTs suggesting significant benefits in OS. These advantageous findings of IR compared to FCR justify conducting an economic evaluation to determine the cost-effectiveness of the drug and subsequent assessment of Conitec. Therefore, if its incorporation into the SUS is viable, IR could be part of the national CLL guideline.
