Introdução: A doença de Pompe é uma doença rara e as terapias visam melhorar a função do coração e dos músculos esqueléticos, além da qualidade de vida dos pacientes. Esta revisão tem como objetivo atualizar e avaliar a segurança e a eficácia da terapia com alfa-glicosidase. Métodos: Foi realizada uma busca nas bases de dados Medline, EMBASE, Center for Reviews and Dissemination (CRD), LILACS e Cochrane. Foram consultadas publicações do National Institute for Health and Care Excellence (NICE) e diretrizes nacionais e internacionais. A qualidade das evidências foi avaliada utilizando os critérios do Grading of Recommendations Assessment, Development and Evaluation - GRADE. Estimativas de custo anuais da alfa-glicosidase para o tratamento de pacientes adultos e pediátricos foram realizadas. Resultados: Em um ensaio clínico randomizado comparando a terapia de substituição enzimática com alfa-glicosidase (20 mg / kg) com placebo por setenta e oito semanas, os resultados favoreceram a alfa-glicosidase (um aumento de 28,1 ± 13,1 m na caminhada de seis minutos e um aumento absoluto de 3,4 ± 1,2 por cento na capacidade vital forçada (CVF), p = 0,03 e p = 0,006, respectivamente). Em outra revisão sistemática, observou-se que pacientes tratados com alfa-glicosidase apresentaram uma taxa de mortalidade cinco vezes menor do que pacientes não tratados (razão de taxa: 0,21; IC 95% 0,11 - 0,41). Em uma população pediátrica com doença avançada, infusões a cada duas semanas prolongaram a sobrevida e a sobrevida sem ventilação invasiva. A qualidade da evidência foi classificada como muito baixa. Custo do tratamento anual: US$ 296.187,64 (paciente adulto com 70 kg) e US$ 42.312,52 (paciente pediátrico com 10 kg). Conclusões: As evidências limitadas para o tratamento da doença de Pompe com alfa-glicosidase sugerem eficácia em pacientes com algumas condições clínicas que não apresentam material imunológico reativo cruzado negativo. O equilíbrio entre a qualidade limitada das evidências e os benefícios demonstrados é favorável, especialmente para a melhora clínica, redução da mortalidade e benefícios intangíveis.

Abstract
Introduction: Pompe disease is a rare disease and therapies aimed at improving the function of the heart and skeletal muscles, and the quality of life of patients. This review aims to update and evaluate the safety and efficacy of alpha-alglucosidase therapy. Methods: Performed search in the databases of Medline, EMBASE, Center for Reviews and Dissemination (CRD), LILACS and Cochrane. Publications of the National Institute for Health and Care Excellence (NICE) and national and international guidelines have been consulted. The quality of the evidence was assessed using the criteria of the Grading of Recommendations Assessment, Development and Evaluation - GRADE. We performed annual cost estimates of alpha-alglucosidase for the treatment of adult and pediatric patients. Results: In a randomized clinical trial comparing alphaalglucosidase enzyme replacement therapy (20 mg / kg) with placebo for seventy-eight weeks, the results favored alpha-alglucosidase (an increase of 28.1 ± 13.1 m in the six minute walk and an absolute increase of 3.4 ± 1.2 percent in forced vital capacity (FVC), p = 0.03 and p = 0.006, respectively). In another systematic review, it was observed that patients treated with alpha-alglucosidase had a mortality rate five times lower than untreated patients (rate ratio: 0.21; 95% CI 0.11 - 0.41). In a pediatric population with advanced disease, biweekly infusions prolonged survival and survival free of invasive ventilation. The quality of the evidence was classified as very low. Annual treatment cost: US$ 296,187.64 (adult patient with 70 kg) and US$ 42,312.52 (pediatric patient with 10 kg). Conclusions: Limited evidence for the treatment of Pompe disease with alpha-algalsidase suggests efficacy in patients with some clinical conditions who do not present negative cross-reactive immune material. The balance between the limited quality of the evidence and the benefits demonstrated is favorable, especially for clinical improvement, reduction of mortality and intangible benefits
