Introdução: Em 2022, o sistema público de saúde brasileiro adotou um limiar explícito de custo-efetividade de US$ 24.232,91 por anos de vida ajustados pela qualidade (QALY) para avaliar tecnologias destinadas ao tratamento de doenças raras. Embora considerado uma estratégia para aumentar a eficiência, a Comissão Nacional de Incorporação de Tecnologias em Saúde (Conitec) recomendou que o limiar não seja usado como parâmetro de knockout. Métodos: Foi realizada uma análise retrospectiva das recomendações da Conitec sobre tecnologias para doenças raras emitidas entre janeiro e outubro de 2023. Os seguintes dados foram extraídos dos relatórios da Conitec: (i) doença (rara ou ultrarrara); (ii) tecnologia em saúde avaliada; (iii) evidências científicas sobre eficácia, efetividade e segurança da tecnologia; (iv) razão de custo-efetividade incremental (ICER); (v) recomendação emitida; e (vi) justificativa para a recomendação. As tecnologias identificadas serão divididas em dois grupos de acordo com a recomendação emitida (positiva ou negativa). A justificativa para cada recomendação, juntamente com outras informações, será revisada para procurar a utilização de limite explícito e seu impacto na tomada de decisão. Resultados: Doze tecnologias para nove doenças raras avaliadas em 2023 foram recuperadas e onze foram incluídas na análise. Seis medicamentos receberam uma recomendação positiva para financiamento. Os ICERs estimados variaram entre ‒ USD 5.278.362,06 para emicizumabe para hemofilia A e USD 116.407,47 para elexacaftor/tezacaftor/ivacaftor para fibrose cística. Dois deles receberam uma recomendação positiva, apesar de seus ICER associados excederem o limite explícito. Todas as recomendações negativas foram associadas a ICERs maiores que o limite, que variou entre USD 52.456,32 e USD 847.942,97. Para dois, o limite foi especificamente mencionado como a justificativa para o negativo. Conclusões: Após a adoção de um limite explícito de custo-efetividade em 2023, a Conitec emitiu duas recomendações positivas de financiamento para tecnologias de doenças raras, apesar de seu ICER associado exceder o limite: agalsidase alfa para doença de Fabry e elexacaftor/tezacaftor/ivacaftor para fibrose cística. Para ambas as recomendações, o impacto favorável do medicamento na história natural da doença foi considerado decisivo.

Abstract
Introduction: In 2022, the Brazilian public health system has adopted an explicit cost-effectiveness threshold of USD24,232.91 per quality-adjusted life years (QALY) for evaluating technologies intended for rare diseases treatment. Although regarded as a strategy for increasing efficiency, the National Committee for Health Tech nology Incorporation (Conitec) has recommendedthatthethreshold should not be used as a knockout parameter. Methods: A retrospective analysis of Conitec’s recommendations regarding technologies for rare diseases issued between January and October 2023 was conducted. The following data were extracted from Conitec’s reports: (i) disease (rare or ultra-rare); (ii) health technology evaluated; (iii) scientific evidence regarding efficacy, effectiveness, and safety of the technology; (iv) incremental cost effectiveness ratio (ICER); (v) recommendation issued; and (vi) rationale for recommendation. Identified technologies will be divided into two groups according to the recommendation issued (positive or negative). The rationale for each recommendation, along with other information, will be reviewed to look for explicit threshold utilization and its impact on decision-making. Results: Twelve technologies for nine rare diseases evaluated in 2023 were retrieved and eleven were included in the analysis. Six drugs have received a positive recommendation for funding. The ICERs estimated varied between‒USD5,278,362.06 for emicizumab for hemophilia A and USD116,407.47 for elexacaftor/tezacaftor/ivacaf tor for cystic fibrosis. Two of them have received a positive recom mendation despite their associated ICER exceeding the explicit threshold. Allnegative recommendationswereassociatedwithICERs higher than the threshold, which varied between USD52,456.32 and USD847,942.97. For two, the threshold was specifically mentioned as the rationale for the negative. Conclusions: After the adoption of an explicit cost-effectiveness threshold in 2023, Conitec has issued two positive funding recom mendations for rare diseases technologies despite their associated ICERexceeding the threshold: agalsidase alpha for Fabry disease and elexacaftor/tezacaftor/ivacaftor for cystic fibrosis. For both recom mendations, the drug’s favorable impact on the natural history of the disease was considered decisive.
