Resumo
Introdução: A bacteremia por Staphylococcus aureus (BSA) representa um grande risco na pediatria, com significativas taxasde morbidade e mortalidade. Devido à complexidade da BSA, os desafios clínicos associados e a possibilidade do tempoprolongado de internação destes pacientes sugere que o ônus financeiro dessa enfermidade pode ser substancial. Objetivo:Analisar a jornada e o custo da doença do atendimento relacionado a BSA, em um hospital pediátrico, na perspectiva do SUScomo fonte pagadora. Material e Método: Foi realizado um estudo de coorte retrospectiva (CAAE: 47556621.0.0000.5580),com mapeamento por local de internação para obter a jornada do paciente e análise bottom-up dos custos de hospitalização,atendidos no sistema público de saúde entre fevereiro de 2014 e agosto de 2021 em um hospital exclusivamente pediátricono Sul do Brasil. Foram coletados apenas os custos médicos diretos (medicamentos, exames e procedimentos realizados einternações, tanto na enfermaria quanto na unidade de terapia intensiva – UTI) do reembolso de cada paciente. Resultados: Aincidência média de BSA na instituição foi de 4 casos ao mês. dos 56 pacientes elegíveis, com idade entre 2 dias e 16 anos,64,3% (n=36) eram do sexo masculino e 34% (n=19) foram admitidos em UTI), mediana de 30 dias de tempo de UTI (5a 259 dias) e mediana de 29,5 dias (9 a 259) de internamento total. O custo total da internação para os 56 pacientes foi deR$1,53 milhões, com mediana de R$6,8 mil (R$427,94 a 237.242,02), média de R$27,3 mil por paciente (DP ± 46,18).Ainda, 16% (n=9) dos pacientes reinfectaram em até 90 dias após fim da antibioticoterapia (custo médio de R$57 mil por reinfecção).Destes que reinfectaram, nenhum fez switch oral. Considerando a jornada do paciente, 29% (n= 16) tiveram sua admissão hospitalarpela emergência SUS (EMERG), 50% (n= 28) tiveram sua internação iniciada por uma enfermaria (ENF), desses, 64% (n=18/28) tiveramalta no posto inicial; 21% (n=12) foram admitidos diretamente na UTI. No total, dos 25 pacientes que frequentaram a UTI, 21 delesestavam na ENF previamente e 16% (n=4) vieram da EMERG. Dois pacientes evoluíram ao óbito na UTI e um pacientenecessitou de hemodiálise (HD) durante o internamento. Conclusões: O tratamento de BSA na pediatria traz uma jornadadinâmica e um custo elevado para o SUS, e ressalta ainda mais a importância do cuidado especializado por meio da equipemultiprofissional. O incentivo a switch oral para completar o tratamento domiciliar pode ser uma alternativa interessante paradesospitalização, redução de reinfecção e custos para o sistema de saúde. Ademais, a análise do custo hospitalar da doençapossibilita para o gestor, juntamente com o delineamento da jornada do paciente, uma reflexão analítica sobre a criação de uma linha de cuidado de BSA, sendo essa uma importante ferramenta de gestão da qualidade da assistência que pode subsidiar a tomada de decisões, oportunizando melhor alocação de recursos.

Abstract
Introduction: Staphylococcus aureus bacteremia (SAB) poses a major risk in pediatrics, with significant morbidity and mortality rates. Due to the complexity of SAB, the associated clinical challenges, and the possibility of prolonged hospitalization of these patients, the financial burden of this disease can be substantial. Objective: To analyze the journey and cost of illness of SAB-related care in a pediatric hospital, from the perspective of SUS as the paying source. Material and Method: A retrospective cohort study (CAAE: 47556621.0.0000.5580) was carried out, with mapping by place of hospitalization to obtain the patient journey and bottom-up analysis of hospitalization costs, attended in the public health system between February 2014 and August 2021 in an exclusively pediatric hospital in southern Brazil. Only direct medical costs (medications, tests and procedures performed and hospitalizations, both in the ward and in the intensive care unit - ICU) were collected from each patient's reimbursement. Results: The average incidence of BSA in the institution was 4 cases per month. Of the 56 eligible patients, aged between 2 days and 16 years, 64.3% (n = 36) were male and 34% (n = 19) were admitted to the ICU), median of 30 days of ICU time (5 to 259 days) and median of 29.5 days (9 to 259) of total hospitalization. The total cost of hospitalization for the 56 patients was R$1.53 million, with a median of R$6.8 thousand (R$427.94 to 237,242.02), and an average of R$27.3 thousand per patient (SD ± 46.18). Furthermore, 16% (n = 9) of the patients were reinfected within 90 days after the end of antibiotic therapy (average cost of R$57 thousand per reinfection). Of those who were reinfected, none underwent oral switch. Considering the patient's journey, 29% (n = 16) were admitted to the hospital via the SUS emergency department (EMERG), 50% (n = 28) had their admission initiated by a ward (ENF), of these, 64% (n = 18/28) were discharged at the initial post; 21% (n = 12) were admitted directly to the ICU. In total, of the 25 patients who attended the ICU, 21 of them had been in the ENF previously and 16% (n=4) came from the EMERG. Two patients died in the ICU and one patient required hemodialysis (HD) during hospitalization. Conclusions: The treatment of BSA in pediatrics involves a dynamic journey and a high cost for the SUS, and further highlights the importance of specialized care by a multidisciplinary team. Encouraging oral switch to complete home treatment may be an interesting alternative for dehospitalization, reducing reinfection and costs for the health system. Furthermore, the analysis of the hospital cost of the disease allows the manager, together with the design of the patient's journey, an analytical reflection on the creation of a BSA care line, which is an important tool for managing the quality of care that can support decision-making, enabling better allocation of resources.
