Resumo
Introdução: No Brasil, estima-se que ocorram 11.540 novos casos de leucemia por ano no triênio de 2023 a 2025. Dentre os tipos de leucemia, destaca-se a Leucemia Linfocítica Crônica (LLC), uma neoplasia hematológica resultante do acúmulo progressivo de linfócitos B maduros, afetando principalmente idosos. A heterogeneidade clínica e biológica, aliada à ausência de estatísticas oficiais específicas, torna o tratamento da LLC complexo. A ausência de um Protocolo Clínico e Diretrizes Terapêuticas (PCDT) específico para LLC no Sistema Único de Saúde (SUS) oportuniza variabilidade nos esquemas terapêuticos utilizados. Objetivo: Esta pesquisa objetivou identificar o histórico de prática clínica terapêutica para LLC em segunda linha no SUS, sob a perspectiva do estado do Rio de Janeiro. Material e Método: Foi realizada pesquisa exploratória na base de dados SIA no período de 2008 a 2023 onde foram buscados os tratamentos referentes ao CID C91.1, prescritos no âmbito do estado do Rio de Janeiro. Os dados foram extraídos dos campos AQ_ESQU_P1 e AQ_ESQU_P2 das APACs de quimioterapia do procedimento 03.04.03.006-6. Resultados: Foram coletados 318 tratamentos, os quais, depois de tratados, revelaram a utilização de 81 esquemas terapêuticos distintos para o tratamento da LLC em 2ª linha no SUS. Sendo a associação de fludarabina com ciclofosfamida a mais registrada (34/297), seguida pela utilização de rituximabe e clorambucil (26/297).

Abstract
Introduction: In Brazil, it is estimated that 11,540 new cases of leukemia will occur per year in the three-year period from 2023 to 2025. Among the types of leukemia, Chronic Lymphocytic Leukemia (CLL) stands out, a hematologic neoplasm resulting from the progressive accumulation of mature B lymphocytes, mainly affecting the elderly. Clinical and biological heterogeneity, combined with the lack of specific official statistics, makes the treatment of CLL complex. The absence of a specific Clinical Protocol and Therapeutic Guidelines (PCDT) for CLL in the Unified Health System (SUS) allows variability in the therapeutic regimens used. Objective: This research aimed to identify the history of clinical therapeutic practice for second-line CLL in the SUS, from the perspective of the state of Rio de Janeiro. Material and Method: An exploratory research was carried out in the SIA database from 2008 to 2023, where treatments related to ICD C91.1, prescribed within the state of Rio de Janeiro, were searched. The data were extracted from the AQ_ESQU_P1 and AQ_ESQU_P2 fields of the chemotherapy APACs of procedure 03.04.03.006-6. Results: 318 treatments were collected, which, after treatment, revealed the use of 81 different therapeutic regimens for the treatment of CLL in the second line in the SUS. The combination of fludarabine with cyclophosphamide was the most registered (34/297), followed by the use of rituximab and chlorambucil (26/297).
