Abstract
Introduction: The growing demand for healthcare services and the limited financial resources available within the Brazilian Unified Health System (SUS) require strategies that combine highquality care with economic sustainability. The outpatient pharmaceutical care clinic serves as a setting for the provision of clinical services by pharmacists, which not only expands access to healthcare but also enables reimbursement for procedures included in the SUS reimbursement schedule. For healthcare managers, measuring and analyzing these data are essential to support decisionmaking and justify the maintenance or expansion of the service. Objective: To evaluate the volume of patient visits, billable procedures, and revenue generated by the outpatient pharmaceutical care clinic of a highcomplexity public hospital, relating these indicators to the hospital’s financial sustainability and service maintenance. Methods: A descriptive, retrospective study was conducted based on billing reports of pharmaceutical consultations performed at the outpatient service of a highcomplexity public hospital in the state of Pará, available in the SUS Outpatient Information System (SIA/SUS). Using outpatient billing records associated with the Brazilian Classification of Occupations (CBO) code No. 223405 (Pharmacist), linked to procedure No. 03.01.01.0048 (Consultation by higher education professionals in specialized care), data on the total number of consultations and the overall revenue generated by pharmaceutical consultations were obtained. Data were organized as absolute values and percentages, without analysis of clinical outcomes. Results: During the study period, the outpatient pharmaceutical care clinic provided 1,043 consultations, of which 24.7% were followup pharmacotherapy consultations (n=258), 53.8% (n=562) were exclusively dedicated to pharmaceutical counseling for access to medicines through the Specialized Component of Pharmaceutical Assistance (CEAF), and 21.3% (n=223) consisted of counseling for preoperative and/or postoperative discharge patients. Billable procedures generated cumulative revenue of R$ 4,806.90, with an average monthly revenue of approximately R$ 437.00. Notably, in the hospital’s 38year history, this was the first time that the pharmacy service generated revenue for the institution. These funds were allocated to maintaining healthcare services, institutional performance bonuses, purchasing materials, and infrastructure investments. Conclusions: The findings demonstrate that the outpatient pharmaceutical care clinic not only fulfills its clinical role but also establishes itself as an emerging strategic asset for the economic sustainability of the hospital studied. The systematic measurement of billing and service provision is fundamental to support management investment decisions, guide expansion policies, and ensure the continuity of this innovative pharmaceutical care model.
