Introdução: A Leucemia Linfocítica Crônica (LLC) é caracterizada pelo acúmulo progressivo de linfóci-tos B monoclonais. Globalmente existem em torno de 191 mil casos e 61 mil mortes por ano atribuídas a LLC. A idade mediana de diagnóstico é em torno de 72 anos. Para a maioria dos casos a LLC é uma doença incurável e os objetivos da terapia são a melhora da qualidade de vida e o prolongamento da sobrevida. Em caso de recidiva sintomática dentro de três anos após terapia de duração fixa ou não res-posta à terapia, o regime terapêutico deve ser alterado, independentemente do tipo de terapia utilizada na 1ª linha de tratamento. Diante desse cenário, é determinante saber se as terapias-alvo (ibrutinibe e venetoclax em monoterapia ou em associação com imunoterapia/quimioimunoterapia) são mais efica-zes e seguras do que a quimioimunoterapia (QIT) para o tratamento da LLC recidivante/refratária (R/R).Métodos: Foi realizada uma revisão sistemática de ensaios clínicos randomizados (ECR) com busca nas bases PUBMED, Embase, COCHRANE e LILACS. Todas as etapas foram conduzidas por dois revisores independentes e não foram aplicados filtros de data e idioma. Para comparar a eficácia e segurança das terapias-alvo (ibrutinibe e venetoclax) com a QIT no tratamento da LLC R/R, foram utilizados os desfechos sobrevida global (SG), sobrevida livre de progressão (SLP) e a frequência de eventos adversos (EA).Resultados: Foram recuperadas 1.208 referências e ao final da seleção, restaram 13 artigos referentes a quatro ECR: RESONATE, HELIOS e NCT01973387 avaliando ibrutinibe; e MURANO avaliando ve-netoclax. A certeza da evidência foi classificada como moderada ou alta para os ECR com maior quan-tidade de pacientes (HELIOS, RESONATE e MURANO). Quanto aos resultados de eficácia para ibruti-nibe, destacam-se um ganho de SLP de 44,1 versus 8,1 meses (HR: 0,148; IC95%: 0,113 – 0,196) quando comparado ao ofatumumabe. Para ibrutinibe associado a bendamustina + rituximabe (BR) versus placebo + BR, observou-se uma maior SLP mediana para o braço ibrutinibe (65,1 meses) do que para o braço placebo (14,3 meses) (HR: 0,229; IC95%: 0,183 – 0,286). Para SG, no RESONATE, foi observada uma SG, censurada para crossover, maior com ibrutinibe do que com ofatumumabe (HR: 0,639; IC95% 0,418 – 0,975). O estudo HELIOS (ibrutinibe + BR versus placebo + BR) observou benefício refletido na SG, ao longo de 60 meses, apesar do crossover ocorrido (HR: 0,611; IC95%: 0,455 – 0,822), porém a mediana de SG não foi alcançada em nenhum dos braços. Já os resultados de eficácia para venetoclax + rituximabe (VR) versus BR, trazem um aumento da SLP (HR: 0,19; IC95%: 0,15 – 0,26) em favor do grupo VR, com uma SLP mediana de 53,6 meses para VR e 17,0 meses para BR. MURANO também evidenciou um aumento da SG (HR: 0,40; IC95%: 0,26 – 0,62), favorecendo o grupo VR. Para a segurança os EA de qualquer grau foram mais frequentes no grupo com ibrutinibe, sendo mais frequentes nos seis primeiros meses. No grupo com venetoclax ocorreu uma frequência maior de neutropenia como EA de qualquer grau, entretanto para os demais EA, como infecção, trom-bocitopenia e anemia, observou-se uma maior frequência no grupo comparador.Discussões e conclusões: A evidência clínica de eficácia (SG e SLP) apresentou superioridade do ibru-tinibe em monoterapia e associado ao BR diante dos comparadores (rituximabe e BR), para pacientes com LLC R/R. O mesmo ocorreu com o VR em comparação ao BR, indicando ser terapias que impactam na sobrevida dos pacientes com LLC R/R. Abstract
Introduction: Chronic Lymphocytic Leukemia (CLL) is characterized by the progressive accumulation of monoclonal B lymphocytes. Globally, there are approximately 191,000 cases and 61,000 deaths per year attributed to CLL. The median age at diagnosis is approximately 72 years. In most cases, CLL is an incurable disease and the goals of therapy are to improve quality of life and prolong survival. In case of symptomatic relapse within three years after fixed-duration therapy or non-response to therapy, the therapeutic regimen should be changed, regardless of the type of therapy used in the first line of treatment. Given this scenario, it is crucial to know whether targeted therapies (ibrutinib and venetoclax as monotherapy or in combination with immunotherapy/chemoimmunotherapy) are more effective and safe than chemoimmunotherapy (QIT) for the treatment of relapsed/refractory (R/R) CLL. Methods: A systematic review of randomized controlled trials (RCTs) was performed by searching the PUBMED, Embase, COCHRANE and LILACS databases. All steps were conducted by two independent reviewers and no date or language filters were applied. To compare the efficacy and safety of targeted therapies (ibrutinib and venetoclax) with CHT in the treatment of R/R CLL, the outcomes overall survival (OS), progression-free survival (PFS), and frequency of adverse events (AE) were used. Results: A total of 1,208 references were retrieved, and at the end of the selection, 13 articles remained referring to four RCTs: RESONATE, HELIOS, and NCT01973387 evaluating ibrutinib; and MURANO evaluating venetoclax. The certainty of the evidence was classified as moderate or high for the RCTs with the largest number of patients (HELIOS, RESONATE, and MURANO). Regarding the efficacy results for ibrutinib, a PFS gain of 44.1 versus 8.1 months (HR: 0.148; 95% CI: 0.113–0.196) stands out when compared to ofatumumab. For ibrutinib combined with bendamustine + rituximab (BR) versus placebo + BR, a longer median PFS was observed for the ibrutinib arm (65.1 months) than for the placebo arm (14.3 months) (HR: 0.229; 95% CI: 0.183–0.286). For OS, in RESONATE, a longer OS, censored for crossover, was observed with ibrutinib than with ofatumumab (HR: 0.639; 95% CI 0.418–0.975). Regarding safety, AEs of any grade were more frequent in the ibrutinib group, being more frequent in the first six months. In the venetoclax group, there was a higher frequency of neutropenia as an AE of any grade, however, for the other AEs, such as infection, thrombocytopenia and anemia, a higher frequency was observed in the comparator group. Discussions and conclusions: The clinical evidence of efficacy (OS and PFS) showed superiority of ibrutinib monotherapy and associated with BR compared to comparators (rituximab and BR), for patients with R/R CLL. The same occurred with VR compared to BR, indicating that these therapies impact the survival of patients with R/R CLL.
