Introdução: Os Acordos de Acesso Gerenciado (ACR) são estratégias para mitigar incertezas relaciona-das às tecnologias de alto custo, como o Zolgensma© (onasemnogeno abeparvoveque), medicamento indicado para o tratamento da Atrofia Muscular Espinhal (AME). O Departamento de Gestão e Incorpo-ração de Tecnologias em Saúde (DGITS) é responsável pelo monitoramento das tecnologias incorpora-das no Sistema Único de Saúde (SUS), incluindo a avaliação e acompanhamento de ACR. Em 2021, foram iniciadas ações para avaliar a viabilidade de ACR para o Zolgensma© no SUS, com a decisão de incorporação, atrelada a um ACR, em 2022. A fim de subsidiar as discussões sobre o ACR no contexto brasileiro, o objetivo desta revisão de literatura foi identificar e descrever informações sobre os acordos já firmados internacionalmente para o Zolgensma© no tratamento da AME.Métodos: Para a identificação dos ACR, foi realizada busca sistematizada no Pubmed utilizando os termos “Managed Entry Scheme” e “Zolgensma”, e seus correlatos. Também foram realizadas buscas manuais no Google, nas agências sanitárias da América Latina e nas agências de Avaliação de Tecnolo-gia em Saúde (ATS) do International Network of Agencies for Health Technology Assessment (INAHTA). As informações sobre os ACR identificados foram sumarizadas utilizando uma planilha contendo infor-mações gerais; indicação; incertezas; desfechos, coleta e análise de dados; e condições econômicas dos acordos. Não houve fonte de financiamento para a realização do estudo.Resultados: Foram identificados quatro ACR para o Zolgensma©, sendo eles da Argentina, Escócia, Espanha e Reino Unido. O ACR da Argentina foi o único identificado para a América Latina, o qual foi firmado em 2023. Todos os acordos tiveram indicação para pacientes com AME 5q, mas houve diferenças entre o grupo de pacientes incluído. Na Argentina, Escócia e Espanha, a indicação foi para pacientes com uma mutação bialélica no gene SMN1 e diagnóstico clínico de AME tipo 1 ou com uma mutação bialélica no gene SMN1 e até 3 cópias do gene SMN2. No Reino Unido, a indicação foi apenas para pacientes pré-sintomáticos com uma mutação bialélica no gene SMN1 e até 3 cópias do gene SMN2. Em relação ao tipo de ACR, na Argentina, foi adotado acordo baseado em desempenho (paga-mento realizado mediante obtenção de desfechos esperados), na Espanha, modelo misto (pagamento por desempenho e acordo de preço por volume) e, na Escócia, apenas desconto simples no preço do medicamento; essa informação não estava disponível para o acordo no Reino Unido. O preço da tecno-logia estava disponível apenas para o ACR da Argentina, sendo ele de 1,3 milhões de dólares.Discussão e conclusões: Foram identificados quatro ACR internacionais para o Zolgensma© no tra-tamento da AME. Entretanto, existem poucas informações disponíveis sobre as características desses acordos, especialmente aquelas relacionadas aos quesitos econômicos e ao preço da tecnologia. Dada a escassez de dados públicos, é possível que existam acordos adicionais não identificados por meio da metodologia de busca utilizada.

Abstract
Introduction: Managed Access Agreements (ACR) are strategies to mitigate uncertainties related to high-cost technologies, such as Zolgensma© (onasemnogeno abeparvovek), a drug indicated for the treatment of Spinal Muscular Atrophy (SMA). The Department of Management and Incorporation of Health Technologies (DGITS) is responsible for monitoring the technologies incorporated into the Unified Health System (SUS), including the evaluation and monitoring of ACR. In 2021, actions were initiated to assess the feasibility of an ACR for Zolgensma© in the SUS, with the decision to incorporate it, linked to an ACR, in 2022. In order to support discussions on the ACR in the Brazilian context, the objective of this literature review was to identify and describe information on the agreements already signed internationally for Zolgensma© in the treatment of SMA. Methods: To identify the ACRs, a systematic search was carried out in Pubmed using the terms “Managed Entry Scheme” and “Zolgensma”, and their correlates. Manual searches were also carried out on Google, in Latin American health agencies and in the Health Technology Assessment (HTA) agencies of the International Network of Agencies for Health Technology Assessment (INAHTA). The information on the identified ACRs was summarized using a spreadsheet containing general information; indication; uncertainties; outcomes, data collection and analysis; and economic conditions of the agreements. There was no source of funding for the study. Results: Four ACRs for Zolgensma© were identified, namely from Argentina, Scotland, Spain, and the United Kingdom. The ACR from Argentina was the only one identified for Latin America, which was signed in 2023. All agreements were indicated for patients with SMA 5q, but there were differences between the group of patients included. In Argentina, Scotland, and Spain, the indication was for patients with a biallelic mutation in the SMN1 gene and a clinical diagnosis of SMA type 1 or with a biallelic mutation in the SMN1 gene and up to 3 copies of the SMN2 gene. In the United Kingdom, the indication was only for presymptomatic patients with a biallelic mutation in the SMN1 gene and up to 3 copies of the SMN2 gene. Regarding the type of ACR, in Argentina, a performance-based agreement was adopted (payment made upon achievement of expected outcomes), in Spain, a mixed model (payment for performance and volume pricing agreement), and, in Scotland, only a simple discount on the price of the drug; This information was not available for the UK agreement. The price of the technology was only available for the Argentine ACR, which was US$1.3 million. Discussion and conclusions: Four international ACRs for Zolgensma© in the treatment of SMA were identified. However, there is little information available on the characteristics of these agreements, especially those related to the economic aspects and the price of the technology. Given the scarcity of public data, it is possible that there are additional agreements not identified through the search methodology used.
