Resumo
Introdução: O linfoma folicular (LF) é um tipo de linfoma não Hodgkin indolente, recidivante e de evolução crônica. O tratamento é principalmente quimioterápico, todavia casos refratários e recidivante têm gerenciamento clínico limitado pela falta de alternativas terapêuticas. Assim, tratamentos inovado-res prometem entregar melhores resultados. Neste sentido, o monitoramento do horizonte tecnológico (MHT), uma etapa específica do processo de avaliação de tecnologias em saúde (ATS), tem como objetivo identificar tecnologias novas e emergentes, prevendo impactos no sistema de saúde, bem como subsidiar as ações dos tomadores de decisão. O objetivo deste trabalho foi identificar tecnologias emergentes para o tratamento do LF, mapeando estado regulatório internacional e nacional, benefícios clínicos, segurança e recomendação internacional pelas agências de ATS.Métodos: Trata-se de um estudo de MHT com busca na plataforma Cortellis Drug Discovery Intelligen-ce, nas agências regulatórias internacionais (FDA e EMA) e nacional (Anvisa); e nas bases de dados Medline e PubMed Central (via PubMed), EMBASE e ClinicalTrials (março de 2023). Foram priorizadas tecnologias com registro há menos de cinco anos no FDA e EMA e ensaios clínicos de fase 2 e 3. Além disso, recomendações das agências de ATS canadense (CADTH), escocesa (SHTG) e inglesa (NICE) foram recuperadas.Resultados: Foram identificadas sete tecnologias (axicabtagene ciloleucel, lisocabtagene e tisagenle-cleucel, mosunetuzumabe, tazemetostato, duvelisibe e zanubrutinibe). Apesar de todas apresentarem registro no FDA e/ou EMA, apenas tisagenlecleucel possui registro na Anvisa. NICE não recomendou o tisagenlecleucel, mosunetuzumabe e duvelisibe; CADTH e SHTG não avaliaram nenhuma das tec-nologias. Foram localizados 13 ensaios clínicos de fase 2 e 6 de fase 3. Axicabtagene foi a terapia com maior taxa de resposta objetiva (94%), seguido por tisagenlecleucel (86%), tazemetostato (77%) e zanubritinibe (68%). Axicabtgene (79%) e tisagenlecleucel (69%) também foram as terapias mais eficazes quando considerada taxa de resposta completa, seguido por duvelisibe (60%). Todos os tra-tamentos, exceto zanubritinibe, reportaram eventos adversos de qualquer grau sendo identificado em 99% a 100% dos participantes dos estudos. Com relação a eventos adversos graves, tisagenlecleucel foi a terapia mais segura (28%), seguido por axicabtagene (46%) e duvelisibe (83%) – demais trata-mentos não reportaram este desfecho.Discussão e conclusões: Os resultados preliminares deste MHT devem ser avaliados com cautela, visto que alguns estudos estão em andamento e foi identificado predomínio de estudos de fase 2 e de braço único. O desfecho mais reportado, taxa de resposta objetiva, é um desfecho substituto, dependente de fatores como tamanho do efeito, duração do efeito e avaliação de risco-benefício. O desfecho avaliado em estudos de braço único não representa uma medida direta de benefício, mas uma avaliação do tumor. Na prática, há grande incerteza na capacidade do desfecho substituto predizer o benefício clí-nico esperado. Esse panorama tecnológico pode apoiar gestores e tomadores de decisão em saúde no acompanhamento de tecnologias emergentes para o tratamento do LF. Por fim, para que ocorra a oferta desses medicamentos no SUS, é necessária a análise pela Conitec, considerando aspectos relacionadas à eficácia, segurança, custo-efetividade e impacto orçamentário para contribuir para a sustentabilidade do sistema de saúde.

Abstract
Introduction: Follicular lymphoma (FL) is a type of indolent, recurrent, and chronic non-Hodgkin lymphoma. Treatment is mainly chemotherapy, but refractory and recurrent cases have limited clinical management due to the lack of therapeutic alternatives. Thus, innovative treatments promise to deliver better results. In this sense, technology horizon monitoring (TMH), a specific stage of the health technology assessment (HTA) process, aims to identify new and emerging technologies, predicting impacts on the health system, as well as supporting the actions of decision makers. The objective of this study was to identify emerging technologies for the treatment of FL, mapping international and national regulatory status, clinical benefits, safety, and international recommendation by HTA agencies. Methods: This is a TMH study with a search on the Cortellis Drug Discovery Intelligence platform, international (FDA and EMA) and national (Anvisa) regulatory agencies; and in the Medline and PubMed Central (via PubMed), EMBASE and ClinicalTrials databases (March 2023). Technologies registered less than five years ago with the FDA and EMA and phase 2 and 3 clinical trials were prioritized. In addition, recommendations from the Canadian (CADTH), Scottish (SHTG) and English (NICE) HTA agencies were retrieved. Results: Seven technologies were identified (axicabtagene ciloleucel, lisocabtagene and tisagenle-cleucel, mosunetuzumab, tazemetostat, duvelisib and zanubrutinib). Although all of them are registered with the FDA and/or EMA, only tisagenlecleucel is registered with Anvisa. NICE did not recommend tisagenlecleucel, mosunetuzumab and duvelisib; CADTH and SHTG did not evaluate any of the technologies. Thirteen phase 2 and 6 phase 3 clinical trials were located. Axicabtagene was the therapy with the highest objective response rate (94%), followed by tisagenlecleucel (86%), tazemetostat (77%) and zanubritinib (68%). Axicabtagene (79%) and tisagenlecleucel (69%) were also the most effective therapies when considering complete response rate, followed by duvelisib (60%). All treatments, except zanubritinib, reported adverse events of any grade being identified in 99% to 100% of study participants. Regarding serious adverse events, tisagenlecleucel was the safest therapy (28%), followed by axicabtagene (46%) and duvelisib (83%) – other treatments did not report this outcome. Discussion and conclusions: The preliminary results of this MHT should be evaluated with caution, since some studies are ongoing and a predominance of phase 2 and single-arm studies was identified. The most reported outcome, objective response rate, is a surrogate outcome, dependent on factors such as effect size, duration of effect and risk-benefit assessment. The outcome assessed in single-arm studies does not represent a direct measure of benefit, but an assessment of the tumor. In practice, there is great uncertainty in the ability of the surrogate outcome to predict the expected clinical benefit. This technological panorama can support health managers and decision makers in monitoring emerging technologies for the treatment of FL. Finally, for these medicines to be offered in the SUS, an analysis by Conitec is necessary, considering aspects related to efficacy, safety, cost-effectiveness and budgetary impact to contribute to the sustainability of the health system.
