Resumo
Introdução: O câncer de mama com superexpressão do fator de crescimento epidérmico humano 2 (HER-2) representa 18 a 30% dos casos dessa doença¹. Atualmente, o Sistema Único de Saúde (SUS) disponibiliza como tratamento o trastuzumabe intravenoso (T IV) e pertuzumabe IV (P IV) associados à quimioterapia citotóxica para pacientes com câncer de mama metastático¹. Outra opção, não disponível no SUS, é o P+T SC, que possui eficácia e segurança semelhante ao P IV + T IV². Sob o olhar do paciente o P+T SC é preferível, pois permite a redução no tempo de administração³.Ademais, a via SC minimiza os riscos de vias de acesso invasivas, como infecções, trombose e desconforto associado ao cateter4. No cenário logístico e financeiro, as formulações SC de imunobiológicos, quando comparados aos medicamentos IV, são responsáveis pela redução do tempo de trabalho dos profissionais da saúde, espera do paciente e custos associados4,5. Objetivo: Descrever e comparar os custos entre P+T SC e o P IV + T IV no tratamento do câncer de mama HER-2+ metastático. Material e Método: Estudo desenvolvido através do método de custeio em saúde. Foi realizado no Hospital de Câncer de Pernambuco (HCP), em junho de 2024, a partir de dados secundários obtidos do sistema informatizado do HCP e relatório da Comissão Nacional de Incorporação de Tecnologias no SUS (CONITEC). Para a execução do estudo foi feita uma comparação entre os gastos reais do protocolo padronizado na instituição (P IV + T IV) e os gastos hipotéticos do P+T SC, considerando a população que realizou P IV + T IV no mês do estudo. Os valores dos protocolos foram produzidos com preços das tecnologias e os insumos utilizados na manipulação e administração. Resultados: A partir do levantamento dos dados, a amostra foi composta por 185 pacientes, com o peso médio de 70 kg e que realizaram um ciclo do protocolo padrão. Logo, o valor unitário deste protocolo foi de aproximadamente R$31.181,05 na dose de ataque e R$17.454,02 na manutenção. Segundo a CONITEC, estima-se que o preço da dose de ataque do P+T SC é R$13.586,44 e R$7.375,64 a manutenção. Considerando que todas as pacientes realizaram o esquema de manutenção com o protocolo padrão, o custo médio mensal aproximado foi R$3.228.993,70. Além disso, foram gastos 277 horas e 30 minutos de infusão. Baseando-se nesse mesmo quantitativo de pacientes, seriam gastos com o uso do P+T SC R$1.364.493,40, com um tempo de infusão de 15 horas e 42 minutos. Assim, estima-se que a incorporação do P+T SC possibilitaria uma economia de R$1.864.500,30 no mês, ademais o tempo gasto com um paciente no protocolo IV permitiria o atendimento de 18 pacientes com o P+T SC. Conclusões: Dessa forma, além da redução de custo direta do P+T SC em relação ao protocolo padrão, esse medicamento apresenta como vantagens a redução do tempo de infusão, que viabiliza o atendimento de um maior número de pacientes, reduzindo seu tempo de permanência no hospital, além de auxiliar na celeridade do fluxo de outros protocolos.

Abstract
Introduction: Breast cancer with overexpression of human epidermal growth factor 2 (HER-2) accounts for 18 to 30% of cases of this disease¹. Currently, the Unified Health System (SUS) provides intravenous trastuzumab (IVT) and IV pertuzumab (IVP) associated with cytotoxic chemotherapy for patients with metastatic breast cancer¹. Another option, not available in the SUS, is P+T SC, which has similar efficacy and safety to IVP + IVT². From the patient's perspective, P+T SC is preferable, as it allows for a reduction in administration time³. Furthermore, the SC route minimizes the risks of invasive access routes, such as infections, thrombosis, and discomfort associated with the catheter4. In the logistical and financial scenario, SC formulations of immunobiologicals, when compared to IV drugs, are responsible for reducing the working time of health professionals, patient waiting times, and associated costs4,5. Objective: To describe and compare the costs between P+T SC and P IV + T IV in the treatment of metastatic HER-2+ breast cancer. Material and Method: Study developed using the health costing method. It was carried out at the Pernambuco Cancer Hospital (HCP), in June 2024, based on secondary data obtained from the HCP's computerized system and a report by the National Commission for the Incorporation of Technologies into the SUS (CONITEC). To carry out the study, a comparison was made between the actual expenses of the standardized protocol at the institution (P IV + T IV) and the hypothetical expenses of P+T SC, considering the population that underwent P IV + T IV in the month of the study. The values of the protocols were produced with the prices of the technologies and the inputs used in the manipulation and administration. Results: From the data collection, the sample consisted of 185 patients, with an average weight of 70 kg and who underwent a cycle of the standard protocol. Therefore, the unit cost of this protocol was approximately R$31,181.05 for the loading dose and R$17,454.02 for the maintenance dose. According to CONITEC, the estimated price of the P+T SC loading dose is R$13,586.44 and R$7,375.64 for the maintenance dose. Considering that all patients underwent the maintenance regimen with the standard protocol, the approximate average monthly cost was R$3,228,993.70. In addition, 277 hours and 30 minutes of infusion were spent. Based on this same number of patients, R$1,364,493.40 would be spent on the use of P+T SC, with an infusion time of 15 hours and 42 minutes. Thus, it is estimated that the incorporation of P+T SC would enable savings of R$1,864,500.30 per month, in addition to the time spent with one patient in the IV protocol allowing the care of 18 patients with P+T SC. Conclusions: Thus, in addition to the direct cost reduction of P+T SC in relation to the standard protocol, this medication has the advantage of reducing the infusion time, which enables the care of a greater number of patients, reducing their length of stay in the hospital, in addition to helping to speed up the flow of other protocols.
