Resumo
Objetivo: Analisar a evolução do impacto orçamentário dos medicamentos biológicos incorporados para artrite reumatóide (AR) ao Sistema Único de Saúde (SUS) entre 2012 a 2016. Métodos: estudo retrospectivo, analítico, de dados secundários em série histórica, sobre a evolução do impacto orçamentário dos medicamentos biológicos incorporados ao SUS para tratamento da AR. O cálculo do Market Share foi obtido da Classificação Internacional de Doenças (CID-10) e Tabela de Procedimentos, Medicamentos, Órtese, Prótese e Materiais Especiais do SUS (SIGTAP). Os recursos do orçamento federal foram extraídos do Sistema Integrado de Planejamento e Orçamento Federal (SIOP) e, os valores das aquisições do Componente Especializado da Assistência Farmacêutica (CEAF) entre 2012 a 2016. Resultados: Os medicamentos biológicos para AR com maior impacto no gasto total dos biológicos para AR do CEAF foram identificados, sendo que o maior gasto foi com o adalimumabe (R$3,8 bilhões) e o menor com o certolizumabe pegol (R$ 50,00 milhões). O adalimumabe também apresentou a maior taxa de crescimento de usuários (1.030 pacientes/ano), com crescimento de 25%, e menor gasto, em torno de R$19,00 milhões/ano e taxa de crescimento negativa de 14%. Para o infliximabe foi observada queda no consumo (-69 pacientes/ano), e impacto crescente de 4% nos recursos do CEAF. O rituximabe teve a menor demanda (745 paciente/ ano), com expansão de 693% e crescimento no dispêndio em 414%. Conclusões: Observou-se a evolução do gasto, com a aquisição de medicamentos biológicos para AR, com significativo impacto econômico para o SUS com a ampliação da oferta de tratamento.

The calculation of Market Share was obtained from the International Classification of Diseases (ICD-10) and Table of Procedures, Medicines, Orthotics, Prosthesis and Special Materials of SUS (SIGTAP). Federal budget funds were drawn from the Federal Integrated Planning and Budget System (Siop), the values of Specialized Component of Pharmaceutical Assistance (CEAF) acquisitions between 2012 to 2016. Results: Biological drugs for AR with the greatest impact on the total expenditure of biologicals for AR under the CEAF were identified, the largest being for adalimumab (R$3.8 billion) ,the lowest for certolizumab pegol (R$ 50.00 million). Aadalimumab had the highest growth rate of users (1,030 patients/year), with growth of 25%, with the lowest expenditure, around R$19.00 million/year and negative growth rate of 14%. IFor infliximab, a drop in consumption was observed (-69 patients/year), and an increasing impact of 4% on CEAF resources. Rituximab showed the lowest demand 745 patients/year, with an expansion of 693%, growth in expenditure by 414%. Conclusions: This study observed the evolution of spending with the acquisition of biological medicines for RA, the significant economic impact for SUS with the expansion of the offer of treatment.
