Resumo
Objetivos: Identificar padrões de tratamento e os custos associados ao tratamento do câncer de mama metastático (MBC) no sistema de saúde privado Brasileiro (BPS). Métodos: Um estudo transversal de revisão de prontuários com coleta de dados de uma clínica oncológica privada no Rio de Janeiro. Foram revisados os prontuários médicos de 32 pacientes com MBC que receberam tratamento sistêmico entre 2000 e 2007, com pelo menos 1 ano de acompanhamento. Resultados: A idade média foi de 56 anos (33 a 73 anos), com 30% HER-2 positivo e 65% receptor hormonal positivo. Os tratamentos de primeira linha mais frequentemente recebidos foram docetaxel (23%), gemcitabina+cisplatina (13%) e paclitaxel (13%). O tratamento de segunda linha mais comum (81% dos pacientes receberam segunda linha) foi docetaxel (16%), gemcitabina+cisplatina (16%) e paclitaxel (16%). A mediana de linhas de tratamento recebidas foi 3 (intervalo: 1 a 8). A utilização de recursos (por mês) incluiu 1,70 visitas ambulatoriais (DP: 1,22), 0,22 radiografias de tórax simples (DP: 0,29), 0,15 tomografias de tórax (DP: 0,24) e 0,14 ultrassonografias abdominais (DP: 0,22). A duração média do tratamento foi de 16,0 meses (DP: 11,6). Os custos médios com medicamentos/materiais, visitas ambulatoriais e exames foram de R$ 106.511, R$ 853 e R$ 3.147, respectivamente. Em média, o tratamento de primeira linha para um paciente com MBC custa R$ 39.367 (DP: R$ 38.121), com uma duração média de tratamento de 5,12 meses (DP: 3,96). Os custos médios do tratamento de segunda linha, com uma duração média de 5,08 meses (DP: 3,23), são de R$ 52.040 (DP: R$ 51.328). Da coorte do estudo, os principais tipos de seguro foram: grupo médico (semelhante a HMO) (41%) e seguradora (28%). Os custos mais altos por paciente foram para aqueles cobertos por seguradoras (R$ 144.432) e custos mais baixos com Cooperativas Médicas (UNIMEDs - R$ 93.771). Conclusões: Pacientes com MBC seguidos por pelo menos um ano após o diagnóstico têm um custo médio de cuidados de R$ 110.512 para o sistema de saúde privado Brasileiro.

Results The average age was 56 years (33 to 73 years old) with 30% HER-2 positive, and 65% hormone receptor positive. First line treatments most frequently received were docetaxel (23%), gemcitabine+cisplatin (13%) and paclitaxel (13%). The most common second line treatment (81% of patients received second line) were docetaxel (16%), gemcitabine+cisplatin (16%), and paclitaxel (16%). The median number of lines of treatment received was 3 (range: 1 to 8). Resource utilization (per month) included 1.70 outpatient visits (SD:1.22), 0.22 plain thorax x-rays (SD:0.29), 0.15 thorax CTs (SD:0.24) and 0.14 abdominal US (SD:0.22). The average duration of treatment was 16.0 months (SD:11.6). The average costs for drugs/materials, outpatients visits and exams were R$106,511, R$853 and R$3,147, respectively. On average, first line treatment for a MBC patient costs R$39,367 (SD: R$38,121), with an average treatment duration of 5.12 months (SD:3.96). The average treatment costs for second line treatment, with an average duration of 5.08 months (SD:3.23) is R$ 52.040 (SD: R$51,328). From the study cohort, the main types of insurance were: medical group (HMO-like) (41%) and insurance company (28%). The higher costs per patients were for those covered under insurance companies (R$144,432) and lower costs with Medical Cooperatives (UNIMEDs - R$93,771). Conclusions Patients with MBC followed for at least one year following MBC diagnosis, result in an average cost of care of R$ 110,512 for the Brazilian private health care system.
