OBJETIVOS: Este estudo teve como objetivo descrever as características sociodemográficas, os padrões de diagnóstico e tratamento, a positividade para HBsAg e a mortalidade associadas à hepatite B crônica (HBC) em adultos brasileiros. MÉTODOS: Foi realizada uma análise descritiva retrospectiva utilizando bases de dados nacionais secundárias e de acesso público do Ministério da Saúde do Brasil, incluindo o Sistema de Informações sobre Agravos de Notificação (SINAN), o Sistema de Controle Logístico de Medicamentos (SICLOM) e o Sistema de Informações sobre Mortalidade (SIM), abrangendo o período de 2010 a 2025. Foram incluídos adultos (≥18 anos) com diagnóstico de HBC (CID-10 B18.0 ou B18.1). Os dados demográficos e clínicos ao diagnóstico, as características do tratamento e os óbitos relacionados à HBC foram analisados por meio de métodos descritivos. RESULTADOS: Entre 2010 e 2023, o SINAN registrou 184.590 brasileiros diagnosticados com hepatite B, dos quais 75,8% (139.856) apresentavam hepatite B crônica (HBC), proporção que se manteve estável ao longo do tempo. A média de idade dos pacientes com HBC foi de 43 anos. A coinfecção por HIV ocorreu em aproximadamente 5,0% dos casos, enquanto cerca de 3,2% apresentavam coinfecção por hepatite C, e 9,2% dos diagnósticos foram identificados durante a gestação. O diagnóstico foi realizado em 99,6% dos pacientes com HBC por meio do teste qualitativo para HBsAg, com 99,0% de positividade, e 50,1% foram testados para HBeAg, dos quais 15,0% foram reagentes. Segundo o SICLOM, 43.405 pacientes estavam em tratamento em 2025, com um aumento médio anual de 5.567 novos inícios de tratamento, sendo a maioria dos novos pacientes não cirróticos (74,6%). O tenofovir foi o tratamento mais frequentemente utilizado em 2025 (50,0%), seguido pelo entecavir e pelo tenofovir alafenamida (45,6% e 4,2%, respectivamente). Apesar disso, em média, 10.547 pacientes interromperam o tratamento anualmente. De 2010 a 2024, o SIM registrou 6.875 óbitos causados ou associados à hepatite B crônica, sendo 2.558 (37,2%) predominantemente associados à cirrose. CONCLUSÕES: Apesar da ampla cobertura diagnóstica e do crescente acesso à terapia antiviral, a hepatite B crônica no Brasil continua resultando em mortalidade significativa, impulsionada principalmente pela cirrose. O fortalecimento do diagnóstico precoce, do tratamento contínuo e do acompanhamento a longo prazo é fundamental para reduzir a carga da hepatite B crônica.

Abstract
OBJECTIVES: This study aimed to describe the sociodemographic characteristics, diagnostic and treatment patterns, HBsAg positivity, and mortality associated with CHB among Brazilian adults.
METHODS: A retrospective descriptive analysis was conducted using separately secondary, publicly available national databases from the Brazilian Ministry of Health, including the Notifiable Diseases Information System (SINAN), the Medication Logistics Control System (SICLOM), and the Mortality Information System (SIM), covering the period from 2010 to 2025. Adults (≥18 years) were diagnosed with CHB (ICD-10 B18.0 or B18.1) included. Demographic and clinical data at diagnosis, treatment characteristics, and CHB-related deaths were analyzed using descriptive methods.
RESULTS: Between 2010-2023, SINAN reported 184.590 Brazilian were diagnosed with hepatitis B, of whom 75.8% (139.856) presented chronic hepatitis B (CHB), a proportion that remained stable over time. The mean age of CHB was 43 years. HIV coinfection occurred in approximately 5.0% of cases, while about 3.2% had hepatitis C coinfection, and 9.2% of diagnoses were identified during pregnancy. Diagnostic was performed in 99.6% of CHB patients by HBsAg qualitative testing, with 99.0% positivity, and 50.1% were tested for HBeAg, of whom 15.0% were reactive. According to SICLOM, 43.405 patients were under treatment in 2025, with an average annual increase of 5.567 new treatment initiations and most new patients were non-cirrhotic (74.6%). Tenofovir was the most frequently treatment in use in 2025 (50.0%), followed by entecavir and tenofovir alafenamide (45.6% and 4.2% respectively). Despite this, in average 10.547 patients interrupted treatment annually. From 2010 to 2024, SIM reported 6.875 deaths caused or associated with chronic hepatitis B were recorded, with 2.558 (37.2%), predominantly associated with cirrhosis.
CONCLUSIONS: Despite broad diagnostic coverage and expanding access to antiviral therapy, chronic hepatitis B in Brazil continues to result in significant mortality, largely driven by cirrhosis. Strengthening early diagnosis, continuous treatment, and long-term follow-up is critical to reducing CHB-related burden.
