Resumo
Objetivos: MDS é uma doença hematológica rara que afeta a produção de células sanguíneas. Um dos objetivos do tratamento é manter a contagem de células sanguíneas em níveis próximos ao normal. Os meios para alcançar isso são feitos principalmente com o uso de fatores de crescimento hematopoético e transfusões. Nosso objetivo foi determinar os custos desse tratamento de suporte sob a perspectiva dos pagadores privados no Brasil, para pacientes com MDS de risco intermediário (INT). Métodos: Adaptamos a árvore de decisão da National Comprehensive Cancer Network (NCCN) para MDS INT à realidade do sistema de saúde Brasileiro. Em seguida, calculamos os custos para cada ramo da árvore, de acordo com os preços locais. Também simulamos os custos totais para uma coorte de 100 pacientes, distribuídos nos ramos de acordo com a epidemiologia esperada. Assumimos um horizonte de um ano de tratamento. Resultados: O custo médio do SC para MDS foi de US$45.006/paciente/ano. Esse valor pode variar de US$33.368 a US$104.210, dependendo das características dos pacientes e dos tipos de tratamentos utilizados. No geral, pacientes que necessitam do uso de imunoterapia, com globulinas anti-timócitos, estavam associados aos custos mais altos. Aqueles que alcançaram a estabilização da doença apenas com o uso de eritropoetina estavam associados aos custos mais baixos. Conclusões: O SC de MDS está associado a um custo médio de US$45.000/paciente/ano no Brasil. Novos tratamentos têm o potencial de mudar esse cenário se puderem diminuir a necessidade dos procedimentos de suporte mais caros.

Abstract
Objetives: MDS is a rare hematological disease, that affects the production of blood cells. One of the goals of the treatment is to maintain the blood-cell count in near-normal levels. The means to achieve this are done mainly with the use of hematopoietic- growth factors and transfusions. Our Objetive was to determine the costs of this supportive treatment under the private payers perspective in Brazil, for patients with intermediate risk (INT) MDS. Methods: We adapted the National Comprehensive Cancer Network (NCCN) for INT MDS decision tree to the reality of the Healthcare system in Brazil. Then, we calculated the costs for each branch of the tree, according to the local prices. We also simulated the total costs for a cohort of 100 patients, distributed in the branches according to the expected epidemiology. We assumed an horizon of one year of treatment. Results: The mean cost of SC for MDS was US$45,006/ patient/ year. This value can vary from US$33,368 to US$104,210, according to the patients’ characteristics and types of treatments used. Overall, patients requiring the use of immunotherapy, with anti-tymocyte globulins were associated with the highest costs. Those achieving disease stabilization solely with the use of erythropoietin were associated with the lowest costs. Conclusions: The SC of SMD is associated with a mean cost of US$45,000/patient/year in Brazil. New treatments have the potential to change this scenario if they can diminish the requirements for the costliest supportive procedures.
