Introdução: A entrega de medicamentos é uma das intervenções mais comuns nas práticas clínicas. Portanto, requer o envolvimento direto dos enfermeiros e o uso de bombas de infusão de alta precisão para aumentar a precisão da infusão. Qualquer erro no processo de entrega de medicamentos pode levar a erros de medicação, resultando em eventos adversos com aumentos consideráveis na duração da internação hospitalar e nos custos. Estudos de pesquisa devem analisar mais essa área em países em desenvolvimento, pois suas realidades diferem muitas vezes profundamente das realidades dos países desenvolvidos, onde a maior parte da literatura dessa área foi desenvolvida. Este estudo analisa essa área no Brasil, um país emergente de destaque. De qualquer forma, a incorporação dessas tecnologias nos serviços de saúde enfrentou dois grandes problemas: as incertezas sobre sua eficácia em reduzir as taxas de eventos adversos relacionados a erros de dosagem durante a infusão e o alto custo de seus insumos. Pergunta: Como as bombas de infusão com biblioteca de medicamentos são custo-efetivas para reduzir as taxas de eventos adversos? Objetivos: Analisar a custo-efetividade e calcular a razão de custo-efetividade incremental do uso de bombas de infusão com biblioteca de medicamentos inteligentes para reduzir eventos adversos durante a entrega intravenosa de medicamentos a pacientes pediátricos e neonatais. Métodos: Custo baseado na estrutura de árvore de decisão, considerando dois cenários como caso base: o de referência, que utiliza bombas de infusão convencionais para realizar a entrega intravenosa de medicamentos com volume superior a 60 ml, e um alternativo, que utiliza bombas de infusão com biblioteca de medicamentos. A análise é feita sob a perspectiva do Sistema Único de Saúde (SUS). As simulações de Monte Carlo abordaram as incertezas da estrutura. A medida de eficácia evitou o evento adverso relacionado ao medicamento. Resultados: As bombas de infusão com biblioteca de medicamentos são mais custo-efetivas do que as bombas de infusão convencionais. A análise probabilística mostrou que as bombas de infusão com biblioteca de medicamentos são mais custo-efetivas do que as convencionais, ratificando o que já havia sido revelado pela curva de aceitação, que demonstrou que as bombas de infusão com biblioteca de medicamentos têm maior probabilidade de serem custo-efetivas em comparação com as bombas de infusão convencionais (com um mínimo de custo-efetividade incremental de USD 1.501,28). Conclusões: O estudo demonstrou que o uso de bombas de infusão com biblioteca de medicamentos na unidade de terapia intensiva pediátrica e neonatal pode melhorar os resultados da estratégia de redução de eventos adversos relacionados a medicamentos.

Abstract
Introduction: The medication delivery is one of the interventions most common in the clinical practices. So, it requires the direct involvement of the nurses and the high precision infusion pumps in order to increase the infusion accuracy. Any mistake in the medication delivery process can lead to the medication error, resulting in the adverse events with considerable increases in the hospital stay length and cost. Research studies should analyze more this area in emerging countries, as their realities differ many times deeply from the realities of developed countries, where most of the literature of this area has been developed. This research study analyses this area in Brazil, a leading emerging country. Anyway, the incorporation of these technologies in health services have clashed in two major problems: the uncertainties of its effectiveness in reducing the adverse drug event rates related to the dose errors during the infusion and the high cost of their inputs. Question: How are drug library infusion pumps cost-effective to reduce the adverse drug event rates? Objectives: To analyze the cost-effectiveness and to calculate the incremental cost-effectiveness ratio of the use of intelligent drug library infusion pumps to reduce the adverse drug event during the intravenous medication delivery to the pediatric and the neonatal patients. Methods: Cost-effect based on the decision-tree framework, considering two scenarios as baseline case: the reference one, which uses the conventional infusion pumps to make the intravenous medication delivery with the volume greater than 60 ml, and an alternative one, which uses the drug library infusion pumps. The analysis is with the Unified Health System (Brazil’s publicly funded health care system) perspective. The Monte Carlo simulations addressed the uncertainties of the framework. The effectiveness measure avoided the adverse drug event. Results: The drug library infusion pumps are more costeffective than the conventional infusion pumps. The probabilistic analysis showed the drug library infusion pumps to be more cost-effective than the conventional ones, ratifying what had already been revealed by acceptance curve, which demonstrated that the drug library infusion pumps are more likely to be cost-effective compared to the conventional infusion pumps (with a minimum of the incremental cost-effectiveness of USD 1.501,28). Conclusions: The study demonstrated that the use of the drug library infusion pumps in the pediatric and the neonatal intensive care unit can improve the results for the adverse drug event reduction strategy
