Resumo
Introdução (com objetivo): O Programa Nacional de Qualificação da Assistência Farmacêutica (QUALIFAR-SUS) busca aprimorar a gestão municipal de medicamentos no SUS. O seu Eixo Estrutura estabelece aos municípios habilitados recursos iniciais para investimentos, seguido de repasses trimestrais de custeio no valor de R$ 6.000,00, sendo estes condicionados à alimentação sistemática da Base Nacional de Dados da Assistência Farmacêutica (BNAFAR) [1,2,4]. Iniciado em 2012, o primeiro ciclo do programa habilitou 453 municípios do Brasil [1,2,3]. A irregularidade ou ausência no envio desses dados técnicos resulta na suspensão do repasse, comprometendo a estruturação das farmácias da atenção primária [1,2]. Objetivos: Analisar a regularidade no recebimento dos repasses federais do eixo estrutura e quantificar a perda financeira acumulada pela não conformidade na alimentação da BNAFAR, em municípios habilitados na primeira etapa do programa no decorrer dos últimos 13 anos. Material e Método: Estudo quantitativo, retrospectivo e longitudinal abrangendo 453 municípios habilitados no Qualifar SUS em 2012. Utilizou-se o Índice de Recebimento de Custeio (IRC), razão entre o valor efetivamente recebido e o teto esperado de R$ 336.000,00 por município, e a perda financeira (Pf), calculada por Pf = 336.000,00 - recursos recebidos no período de 2012 a 2025. Os municípios foram classificados em quatro faixas de regularidade: Excelência (IRC =100%), Alto (IRC 75-99%); Médio (50-74%); Baixo (25-49%) e Muito baixo (0-24%). Resultados: Na amostra, 20% (n=90) dos municípios obtiveram IRC de excelência, 47% (n=211) alto, 19% (n=85) médio, 12% (n=56) baixo e 3% (n=14) muito baixo. Municípios da região Nordeste (81%) obtiveram IRC superiores à média nacional (78%), enquanto que os da região Norte foram de 52%. O coeficiente de Pearson entre IDHM e IRC indicou correlação positiva fraca (r ≈ 0,16; p < 0,05). A perda financeira global no período somou R$ 33.029.136,00. Conclusões: O estudo evidencia o alto desempenho de 67% dos municípios (faixas de Excelência e Alto IRC), ratificando o papel indutor do QUALIFAR-SUS na consolidação da gestão farmacêutica local. Mas também revela lacuna crítica entre o financiamento planejado e o distribuído, com uma perda de recursos no período estudado (13 anos), que equivale ao custeio de 98 municípios,que deveriam ser investidos na estruturação de centenas de farmácias da atenção primária do SUS, beneficiando a população. A fraca correlação com o IDHM indica que o desenvolvimento socioeconômico não é o principal preditor da não conformidade com o BNAFAR, levantando a hipótese de que a perda de recursos do Eixo Estrutura do QUALIFAR-SUS é um problema de governança de dados e continuidade administrativa. Isso exige um aprofundamento qualitativo para revelar quais estratégias de apoio técnico que transcendam o aporte financeiro podem ser ampliadas para garantir a sustentabilidade do Eixo Estrutura no SUS pelos municípios.

Abstract
Introduction (with objective): The National Program for the Qualification of Pharmaceutical Assistance (QUALIFAR-SUS) seeks to improve the municipal management of medicines in the SUS (Brazilian Unified Health System). Its Structure Axis provides eligible municipalities with initial resources for investments, followed by quarterly cost-sharing payments of R$ 6,000.00, which are conditional on the systematic updating of the National Pharmaceutical Assistance Database (BNAFAR) [1,2,4]. Initiated in 2012, the first cycle of the program enabled 453 municipalities in Brazil [1,2,3]. Irregularity or absence in the submission of this technical data results in the suspension of the transfer, compromising the structuring of primary care pharmacies [1,2]. Objectives: To analyze the regularity in the receipt of federal transfers from the structure axis and to quantify the accumulated financial loss due to non-compliance in updating the BNAFAR, in municipalities enabled in the first stage of the program over the last 13 years. Material and Method: This was a quantitative, retrospective, and longitudinal study encompassing 453 municipalities qualified under the Qualifar SUS program in 2012. The Cost Receipt Index (CRI) was used, calculated as the ratio between the amount actually received and the expected ceiling of R$ 336,000.00 per municipality, and the financial loss (FL) was calculated as FL = 336,000.00 - resources received in the period from 2012 to 2025. The municipalities were classified into four regularity ranges: Excellent (CRI = 100%), High (CRI 75-99%); Medium (50-74%); Low (25-49%) and Very Low (0-24%). Results: In the sample, 20% (n=90) of the municipalities obtained an excellent IRC, 47% (n=211) high, 19% (n=85) medium, 12% (n=56) low, and 3% (n=14) very low. Municipalities in the Northeast region (81%) obtained IRCs higher than the national average (78%), while those in the North region were 52%. The Pearson coefficient between HDI and IRC indicated a weak positive correlation (r ≈ 0.16; p < 0.05). The overall financial loss in the period totaled R$ 33,029,136.00. Conclusions: The study highlights the high performance of 67% of the municipalities (Excellent and High IRC ranges), ratifying the inductive role of QUALIFAR-SUS in consolidating local pharmaceutical management. However, it also reveals a critical gap between planned and distributed funding, with a loss of resources over the study period (13 years) equivalent to the operating costs of 98 municipalities, which should have been invested in structuring hundreds of primary care pharmacies within the SUS (Brazilian Unified Health System), benefiting the population. The weak correlation with the HDI (Human Development Index) indicates that socioeconomic development is not the main predictor of non-compliance with the BNAFAR (National Basic Assessment of Primary Care), raising the hypothesis that the loss of resources from the Structure Axis of QUALIFAR-SUS (Quality Assessment of Primary Care for the Unified Health System) is a problem of data governance and administrative continuity. This requires further qualitative analysis to reveal which technical support strategies that transcend financial contributions can be expanded to ensure the sustainability of the Structure Axis within the SUS by municipalities.
