Resumo
Objetivos: Avaliar a relação custo-efetividade da cirurgia aberta (OPBARS) e da cirurgia laparoscópica Roux-en-Y (LAPBARS) em comparação com o tratamento não cirúrgico convencional (CONVT) para pacientes obesos, com acompanhamento de 5 anos, sob a perspectiva do pagador privado Brasileiro. Métodos: Foi construído um modelo analítico de árvore de decisão para estimar custos e desfechos entre OPBARS e LAPBARS versus CONVT, medindo perda de peso, resolução de comorbidades (diabetes tipo 2 e hipertensão; Buchwald 2009) e o impacto disso em QALYs para pacientes obesos com IMC >35. Para avaliar as diferenças entre cirurgia e tratamento CONV, assumiu-se a mesma eficácia para OPBARS e LAPBARS. A diretriz Brasileira para Avaliação de Tecnologias em Saúde (Vianna, 2007) foi o guia para o desenvolvimento do estudo. Um painel de especialistas foi realizado para obter práticas locais desde a avaliação pré-operatória até a última visita médica 5 anos após o procedimento. Foi aplicada uma técnica de microcusteio com base em listas de preços públicos (SIMPRO 2009, CBHPM 5ª Ed) para valorizar o uso dos recursos. Apenas custos médicos diretos foram considerados. Foi assumida uma taxa de desconto de 5% para custos e desfechos. Foi realizada uma análise de sensibilidade univariada para verificar a robustez dos resultados. Resultados: A resolução mais sustentável das comorbidades para OPBARS e LAPBARS resultou em um aumento de 1,34 QALYs em 5 anos em comparação com CONVT (3,26 QALYs vs. 1,92 QALYs). Os custos totais para os 5 anos foram de R$ 26.456 para OPBARS, R$ 32.515 para LAPBARS e R$ 19.217 para CONVT. Portanto, a razão de custo-efetividade incremental foi de R$ 7.449 por QALY para OPBARS e R$ 10.395 por QALY para LAPBARS quando comparado com CONVT. Conclusões: Os resultados sugerem que OPBARS e LAPBARS são escolhas mais seguras e custo-efetivas para o tratamento da obesidade e resolução de comorbidades, sob o sistema de saúde privado Brasileiro.

Abstract
Objetives: To assess the cost-effectiveness of the open (OPBARS) and the laparoscopic surgery Roux-en-Y gastric bypass (LAPBARS) versus the conventional non-surgical treatment (CONVT) for obese patients, in 5 years of follow up, under the Brazilian private payer perspective. Methods: An analytic decision-tree model was built to estimate costs and outcomes among OPBARS and LAPBARS versus CONVT, measuring weight loss, co morbidities resolution (T-2 diabetes and hypertension; Buchwald 2009) and the impact of that on QALYs for obese patients with BMI>35. In order to evaluate the differences of surgery against CONV treatment we assumed the same efficacy for OPBARS and LAPBARS. The Brazilian guideline for Health Technology Assessment (Vianna, 2007) was the driver for the study development. A panel of specialists was conducted to obtain local practice from pre-operatory evaluation to the last medical visit 5 years after the procedure. A micro costing technique based on public price lists (SIMPRO 2009, CBHPM 5o Ed) was applied to value the resources usage. Only direct medical costs were considered. A discount rate of 5% was assumed for costs and outcomes. One-way sensitivity analysis was performed to check the robustness of the Results. Results: The more sustainable resolution of the co morbidities for OPBARS and LAPBARS resulted in an increase of 1.34 QALYs in 5 years versus CONVT (3.26 QALYs vs. 1.92 QALYs). The total costs for the 5 years were R$ 26, 456 for OPBARS, R$ 32,515 for LAPBARS and R$ 19,217 for CONVT. So, the incremental cost-effectiveness ratio was R$ 7,449 per QALY for OPBARS and R$ 10,395 per QALY for LAPBARS when compared with CONVT. Conclusions: Findings suggest OPBARS and LAPBARS as safer and cost-effective choices for obesity treatment and co morbidity resolution, under the Brazilian private healthcare system.
