Objetivos: A doença renal crônica (DRC) está associada a níveis anormalmente elevados de fosfato no sangue, o que contribui para a presença de calcificações vasculares, aumentando a probabilidade de ocorrência de eventos cardiovasculares e morte nesses pacientes. O objetivo desta análise foi avaliar a relação custo-efetividade incremental do uso de sevelamer para o manejo da hiperfosfatemia no Brasil. Métodos: Um modelo Markov foi criado para estimar os custos e benefícios mensais do tratamento com sevelamer ou comprimidos de cálcio em pacientes com insuficiência renal, considerando um horizonte temporal de 60 meses. As probabilidades de transição foram obtidas de ensaios clínicos identificados por meio de uma revisão sistemática da literatura. A medida de eficácia considerada foi o aumento na sobrevida do paciente (meses). Apenas custos diretos foram considerados. Os custos foram calculados com preços de 2009 e são expressos em dólares americanos. Além disso, foram realizadas análises de sensibilidade univariada e mudanças de cenários. A taxa de desconto foi de 5%. Resultados: O custo esperado foi de US$37.477 para cálcio e US$58.397 para sevelamer. Pacientes no grupo de cálcio sobreviveriam por 51 meses, em comparação com o grupo de sevelamer (54 meses). A Razão de Custo-Efetividade com cálcio foi de US$734 e com sevelamer foi de US$1.073, respectivamente, e a Razão de Custo-Efetividade Incremental para a implementação de sevelamer vs. cálcio foi de US$6.135 (1 USD = 1,8 BRL). Conclusões: Sevelamer é um medicamento custo-efetivo para o tratamento da hiperfosfatemia em pacientes com DRC no contexto Brasileiro.

Abstract
Objetives: The chronic kidney disease (CKD) is associated with an abnormally elevated level of phosphate in the blood, which contributes to the presence of vascular calcifications, thus increasing the probability of the occurrence of cardiovascular events and death in these patients. The Objetive of this analysis was to evaluate the incremental cost-effectiveness of the use of sevelamer to manage hyperphosphatemia in Brazil Methods: A Markov model was created to estimate the monthly costs and benefits of the treatment with sevelamer or calcium tablets in patients with renal failure considering a temporary horizon of 60 months. The transition probabilities were taken from clinical trials identified through a systematic review of literature. The effectiveness measure considered was an increase in patient survival (months). Only direct costs were considered . Costs were calculated using 2009 prices and are expressed in US dollars. In addition, univariate sensitivity analysis and scenario changes were performed. The discount rate was 5%. Results: The expected cost was US$37,477 for calcium and US$58,397 for sevelamer. Patients in calcium group would survive 51 months, compared to the sevelamer group (54 months). The Cost-Effectiveness Ratio with calcium was $734 and with sevelamer was $1073 respectively, and the incremental Cost-Effectiveness Ratio for the implementation of sevelamer vs. calcium was $6135 .(1 USD= 1.8 BRL$) Conclusions: Sevelamer is a cost-effective drug for the treatment of hyperphosphatemia in patients with CKD in the brazilian context.
