Resumo
Objetivos: O objetivo do estudo foi comparar a eficácia e os custos de tratamento do FLUCAM versus FCR no tratamento de segunda linha de CLL em um horizonte temporal de um ano, sob a perspectiva do sistema de saúde privado Brasileiro (BPS). Métodos: Foi realizada uma revisão da literatura e encontrados dois estudos de fase II, avaliando a eficácia do FLUCAM (JCO 2005;23:7024) ou FCR (JCO 2005;23:4070). O FLUCAM apresentou taxas de resposta mais altas e menor sobrevida global e tempo até a progressão da doença em comparação com o FCR. Agrupando pacientes de ambos os estudos em grupos de risco e assumindo sobrevivências globais medianas de 11, 6 e 2 anos para pacientes de baixo, intermediário e alto risco, respectivamente, resultou em uma sobrevida global mediana esperada de 2,89 anos para o FLUCAM e 4,15 anos para o FCR. Isso reflete a maior gravidade da doença na população estudada com FLUCAM no início do estudo. Portanto, não é possível concluir por meio de uma comparação indireta que um tratamento é superior ao outro. Assumindo eficácias equivalentes para ambos os tratamentos, foi desenvolvida uma análise de minimização de custos. Os protocolos dos estudos foram valorizados considerando uma superfície corporal média de 1,7 m² e preços máximos ao consumidor para os medicamentos. Profilaxia antimicrobiana e medicamentos pré-tratamento foram incluídos. O número médio de ciclos administrados foi de 4,2 ciclos para ambos os protocolos. Reduções de dose foram consideradas para FCR. Oitenta e seis por cento dos pacientes receberam dosagem normal, 13% tiveram redução de dose de nível 1 a partir do terceiro ciclo, e 1% teve redução de nível 1 no ciclo 3 e redução de nível 2 a partir do ciclo 4. Resultados: Os custos de tratamento encontrados em um ano foram de R$ 36.370 para FLUCAM e R$ 63.778 para FCR, representando uma redução de custo de R$ 27.408 (US$ 11.874) a favor do FLUCAM. Conclusões: Assumindo eficácias equivalentes para ambos os tratamentos, o FLUCAM apresentou um custo total de tratamento inferior ao FCR, sendo uma alternativa valiosa diante das restrições orçamentárias para o BPS.

Abstract
Objetives: The Objetive of the study was to compare the efficacy and treatment costs of FLUCAM versus FCR in the second line treatment of CLL in a one year time horizon under the Brazilian private health care system (BPS) perspective. Methods A literature review was performed and two phase II trials were found, evaluating the efficacy of FLUCAM (JCO 2005;23:7024) or FCR (JCO 2005;23:4070). FLUCAM presented higher response rates and lower overall survival and time to disease progression than FCR. Grouping patients from both studies into risk groups, and assuming median overall survivals of 11, 6 and 2 years for low, intermediate and high risk patients, respectively, resulted in an expected median overall survival of 2.89 years for FLUCAM and 4.15 years for FCR. This reflects the higher disease severity in the FLUCAM study population at baseline. Therefore, it cannot be concluded through an indirect comparison that one treatment is superior to the other. Assuming equivalent efficacies for both treatments, a cost-minimization analysis was developed. Study protocols were valued considering an average body surface of 1.7m2 and maximum prices to consumer for drugs. Antimicrobial prophylaxis and pre-treatment drugs were included. The average number of cycles administered was 4.2 cycles for both protocols. Dose reductions were considered for FCR. Eighty-six of patients have received normal dosage, 13% had a level 1 dose reduction starting on the third cycle, and 1% had a Level 1 reduction at Cycle 3 and a Level 2 reduction from Cycle 4. Results Treatment costs found in one year were R$36,370 for FLUCAM and R$63,778 for FCR, representing a cost reduction of R$27,408 (US$11,874) in favor of FLUCAM. Conclusions Assuming equivalent efficacies for both treatments, FLUCAM presented a lower total treatment cost than FCR, being a valuable alternative in face of budget constraints, for the BPS.
