Objetivo: Desenvolver um estudo econômico comparando os custos anuais relacionados ao tratamento de pacientes com diabetes mellitus tipo 2 (DM2) utilizando novos análogos de insulina basal, sob a perspectiva do Sistema de Saúde Privado Brasileiro. Métodos: Os dados clínicos foram obtidos a partir de um estudo multicêntrico randomizado, aberto e paralelo de 52 semanas, comparando a eficácia e segurança das insulinas Glargina (GI) e Detemir (DI) como complemento aos medicamentos orais para redução da glicose em pacientes com DM2. Os preços das insulinas e outros recursos médicos (consumo de agulhas e tiras de teste de glicose no sangue) foram obtidos de bases de dados públicas e tarifas publicadas. Os cálculos de custo referem-se ao ano de 2007, atribuídos em Reais Brasileiros (BRL) e Dólares Americanos (Paridade de Poder de Compra de 2005: 1USD = 1,4 BRL). Resultados: O estudo demonstrou que pacientes tratados com GI necessitaram de uma dose diária de insulina 37% menor do que aqueles tratados com DI (26,09 UI vs. 29,32 UI, para doses uma vez ao dia, ou 51,55 UI/kg para doses duas vezes ao dia) para atingir uma glicemia de jejum ≤110 mg/dl. Em pacientes com DM2, o tratamento anual com GI apresentou custos totais inferiores em comparação com DI (BRL 4442 vs. BRL 5391; custo incremental de BRL 949, equivalente a US$ 678), o que permite economias de até 18%. As economias estão relacionadas aos custos de insulina, agulhas e tiras de teste de glicose no sangue. Conclusões: Para pacientes com DM2, o tratamento anual com GI está associado a custos totais anuais menores e permite economias de até BRL 949 (US$ 678) por paciente-ano, correspondendo a uma economia anual de 18%.

Abstract
Objetives: In Brazil there are several insulins for the treatment of type 2 diabetes mellitus (DM2), with different profiles and costs. The Objetive was to develop an economic study comparing annual costs related to the treatment of DM2 patients with new basal insulin analogues, under the Brazilian Private Health Care System perspective. Methods Clinical data related with each treatment derives from a 52-week randomised, open-label, parallel, multinational trial, which compares efficacy and safety of Glargine insulin (GI) and Detemir insulin (DI) as a supplementation of oral glucose-lowering drugs in DM2 patients. Data about the prices of insulins and other related medical resources (consumption of needles and blood glucose test strips) have been obtained from public databases and published tariffs. Cost calculations refer to year 2007 and were attributed in Brazilian Reais (BRL) and US dollars (2005 Purchasing Power Parity index 1USD=1.4BRL) Results The trial has shown that patients treated with GI required a 37% lower daily dose of insulin than those patients treated with DI (26.09 IU vs 29.32 IU, for once a day doses, or 51.55 IU/Kg, for twice a day doses) to target a fasting blood glucose ≤110mg/dl. In patients with DM2, annual treatment with GI has lower total costs than DI (BRL4442 vs. BRL5391; incremental cost of BRL949, equivalent to US$678), which allows savings up to 18%. Savings are related to costs of insulin, needles and blood glucose test strips. Conclusions For DM2 patients, annual treatment with GI is associated with lower annual total costs and allows savings up to BRL949 (US$678) per patient-year corresponding to annual savings of 18%.
