Resumo
Objetivos: Desenvolver uma análise de custo-efetividade e impacto orçamentário do uso de hialuronato de sódio intra-articular 25mg/2,5ml (SH) versus artroplastia total do joelho (ATJ) no tratamento de osteoartrite do joelho graus II e III da classificação de Kellgren-Lawrence, em pacientes que falharam no tratamento conservador, sob a perspectiva do sistema público de saúde Brasileiro (SUS). Métodos: Um modelo Markov foi desenvolvido para projetar custos e desfechos associados à progressão da osteoartrite do joelho em 3 anos. No modelo, todos os pacientes eram elegíveis para cirurgia e podiam receber SH ou se submeter a ATJ imediatamente. O SH poderia adiar ou cancelar a cirurgia. A duração do ciclo foi de 3 meses e as probabilidades de transição correspondentes foram obtidas da literatura. O desfecho foi expresso como o número de ATJs evitadas. A análise considerou apenas custos médicos diretos, incluindo medicamentos, consultas médicas, procedimentos e tratamento de complicações. Custos e desfechos foram descontados a uma taxa de 5% ao ano. Uma análise de impacto orçamentário foi desenvolvida considerando o número estimado de ATJs no SUS em 2009. Parâmetros principais foram avaliados em uma análise de sensibilidade. Resultados: Em um horizonte de 3 anos, o custo total por paciente foi de US$3445 (índice PPP de 2005 USD1,0 = R$1,4) para o grupo SH e R$4381 para o grupo ATJ. A redução no número absoluto de cirurgias foi de 72,8% no grupo SH, em comparação com ATJ. Da mesma forma, houve uma redução de custo de US$936, a favor do SH. Considerando 3991 pacientes elegíveis no sistema público, os custos anuais totais para os grupos SH e ATJ foram de US$5.136.234 e US$17.484.003, respectivamente. Como resultado, o impacto orçamentário foi de US$12.347.769, a favor do SH. A análise de sensibilidade sobre o horizonte de tempo mostrou um resultado de economia de custos até 4 anos. Conclusões: O uso de SH após falha no tratamento conservador em pacientes com osteoartrite do joelho graus II ou III mostrou ser eficaz, levando à redução de custos e número de cirurgias.

Abstract
Objetives: To develop a cost-effectiveness and budget impact analysis of intra-articular sodium hyaluronate 25mg/2.5ml (SH) versus total knee arthroplasty (TKA) in the treatment of knee osteoarthritis grades II and III of Kellgren-Lawrence classification, in patients who failed conservative treatment, under the Brazilian public healthcare system (BPHS) perspective. Methods: A Markov model was developed to project costs and outcomes associated with knee osteoarthritis progression in 3 years. In the model all patients were eligible to surgery and could either receive SH or undergo TKA immediately. SH could delay or cancel surgery. The cycle duration was 3 months and the corresponding transition probabilities were obtained from the literature. The outcome was expressed as the number of avoided TKAs. The analysis considered only direct medical costs, including drugs, physician visits, procedures and complications treatment. Costs and outcomes were discounted at 5% per year. A budget impact analysis was developed considering the estimated number of TKAs in the BPHS in 2009. Main parameters were evaluated in a sensitivity analysis. Results: In a 3-year time horizon, total costs per patient were US$3445 (2005-PPP-index USD1.0=R$1.4) for the SH group and R$4381 for the TKA group. The reduction in the absolute number of surgeries was 72.8% in the SH group, compared with TKA. Likewise, there was a cost reduction of US$936, in favor of SH. Considering 3991 eligible patients in the public system, the total annual costs for SH and TKA groups were US$5,136,234 and US$17,484,003, respectively. As a result, the budget impact was US$12,347,769, in favor of SH. The sensitivity analysis about the time horizon showed a cost-saving result until 4 years. Conclusions: The use of SH after failure of conservative treatment in patients with grades II or III knee osteoarthritis showed to be effective, leading to a reduction in costs and number of surgeries.
